(Medicines.org.uk)

ZYMRON (Mirtazapine) 15 mg 30 Tablets

Mirtazapine 15 mg is a tetracyclic antidepressant indicated for the treatment of major depressive episodes in adults. It enhances central noradrenergic and serotonergic neurotransmission and has clinically relevant H1-antihistaminic activity, which contributes to sedation and increased appetite. (Medicines.org.uk)

Mechanism of Action

Mirtazapine acts primarily as a central presynaptic α2-adrenergic antagonist, thereby increasing noradrenergic and serotonergic neurotransmission.

It also antagonises 5-HT2 and 5-HT3 receptors and has significant H1-histamine receptor antagonism.

Antimicrobial Spectrum

None.

Mirtazapine has no antibacterial, antiviral, or antifungal activity.

Pharmacokinetics

Mirtazapine is well absorbed after oral administration, with oral bioavailability of approximately 50%.

Peak plasma concentrations are generally reached within approximately 2 hours, and plasma protein binding is approximately 85%.

It undergoes hepatic metabolism involving CYP1A2, CYP2D6, and CYP3A4 and is eliminated in urine and faeces.

The elimination half-life is generally 20–40 hours, allowing once-daily administration. (Medicines.org.uk)

Contraindications

Contraindicated in patients with hypersensitivity to Mirtazapine or any excipient.

Concomitant use with monoamine oxidase inhibitors (MAOIs) is contraindicated because of the risk of serotonin syndrome.

Precautions

Patients should be monitored for clinical worsening, suicidal ideation, and suicidal behaviour, particularly during early treatment and following dose changes.

Mirtazapine may cause substantial somnolence and dizziness.

Use with caution in epilepsy, hepatic or renal impairment, cardiovascular disease, hypotension, glaucoma, urinary retention, and diabetes.

Rare but serious agranulocytosis and neutropenia have been reported.

Hyponatraemia and SIADH may occur, particularly in older or predisposed patients.

Treatment should generally be tapered rather than stopped abruptly. (Medicines.org.uk)

Use During Pregnancy and Breastfeeding

Pregnancy: Available clinical data are limited. Treatment should be used when the expected benefit justifies the potential risk.

Breastfeeding: Mirtazapine is excreted into breast milk in small amounts; use should be based on an individual benefit-risk assessment.

Adverse Effects

Common adverse effects include somnolence, increased appetite, weight gain, dry mouth, dizziness, fatigue, and headache.

Other reported effects include constipation, peripheral oedema, orthostatic hypotension, tremor, abnormal dreams, and nausea.

Serious but uncommon reactions include agranulocytosis, severe neutropenia, serotonin syndrome, hyponatraemia, seizures, mania/hypomania, and severe cutaneous reactions.

Overdose

Overdose may cause marked sedation, confusion, tachycardia, blood-pressure abnormalities, and CNS depression.

There is no specific antidote. Treatment is supportive and symptomatic.

Drug Interactions

Concomitant use with MAOIs is contraindicated.

Caution is required with other serotonergic medicines such as SSRIs, SNRIs, tramadol, triptans, lithium, linezolid, and St. John’s Wort.

Alcohol, benzodiazepines, opioids, sedating antihistamines, and other CNS depressants may increase sedation.

CYP3A4 inducers such as carbamazepine and phenytoin may reduce Mirtazapine concentrations, whereas strong CYP3A4 inhibitors may increase exposure.

Drug Name and Strength

Mirtazapine 15 mg per tablet

Mirtazapine 15 mg is available in both film-coated and orodispersible tablet formulations depending on the product. Thai FDA records list several 15 mg products, including ZYMRON 15 MG and REMERON SOLTAB 15 MG as orodispersible tablets. (ข้อมูลยาแห่งชาติ)

Indications

Indicated in adults for the treatment of episodes of major depression. (Medicines.org.uk)

Recommended Dosage

The usual adult starting dose is:

15 mg or 30 mg once daily

The effective daily dose is generally 15–45 mg/day.

Because of its relatively long half-life and sedative effect, Mirtazapine is commonly administered once daily at night. (Medicines.org.uk)

Clinical effects generally begin within 1–2 weeks, with an adequate response usually expected within 2–4 weeks.

If response is insufficient, the dose may be increased up to a maximum of 45 mg/day.

Patients who respond should generally continue treatment for at least 6 months to maintain remission. (Medicines.org.uk)

Warnings in Accordance with Public Safety Announcements

Patients should be monitored for suicidal thoughts or behaviour, particularly during early treatment and after dose adjustments.

Fever, sore throat, oral ulcers, or other signs of infection require prompt assessment because rare agranulocytosis or severe neutropenia may occur.

Symptoms compatible with serotonin syndrome require urgent assessment.

Treatment should generally be tapered gradually rather than stopped abruptly.

Because Mirtazapine may cause substantial somnolence, patients should avoid driving or operating machinery until they know how the medicine affects them.

Storage Conditions

Storage requirements depend on the specific Mirtazapine product. Follow the marketed product label.

In general, keep the medicine in the original packaging, protected from excessive moisture, heat, and light.
