(Medicines.org.uk)

Mirtazapine 15 mg is an antidepressant indicated for the treatment of major depressive disorder in adults. It enhances central noradrenergic and serotonergic neurotransmission and has clinically relevant H1-histamine receptor antagonistic activity, which contributes to sedation and increased appetite. The Thai-registered REMERON SOLTAB 15 MG is an orodispersible tablet containing Mirtazapine 15 mg. (ข้อมูลยาแห่งชาติ)
Mirtazapine is a centrally active presynaptic α2-adrenergic antagonist that enhances central noradrenergic and serotonergic neurotransmission.
It blocks 5-HT2 and 5-HT3 receptors, thereby favouring serotonergic neurotransmission through 5-HT1 receptors, and also has significant H1-histamine receptor antagonistic activity. (Medicines.org.uk)
None.
Mirtazapine has no antibacterial, antiviral, or antifungal activity.
Mirtazapine is rapidly and well absorbed after oral administration, with an oral bioavailability of approximately 50%. Peak plasma concentrations are reached after approximately 2 hours, and food does not materially affect its pharmacokinetics. (Medicines.org.uk)
Plasma protein binding is approximately 85%. Metabolism involves CYP1A2, CYP2D6, and CYP3A4.
The elimination half-life is generally approximately 20–40 hours, supporting once-daily administration.
Contraindicated in patients with hypersensitivity to Mirtazapine or any excipient.
Concomitant use with monoamine oxidase inhibitors (MAOIs), or use within 14 days after stopping an MAOI, is contraindicated. An MAOI should likewise not be started until at least 14 days after Mirtazapine is discontinued. (DailyMed)
Patients should be monitored for clinical worsening, suicidal thoughts, and suicidal behaviour, particularly during early treatment and after dose adjustments. (Organon)
Mirtazapine may cause significant somnolence, dizziness, and impaired alertness.
Rare neutropenia and agranulocytosis have been reported. Fever, sore throat, oral ulcers, or other signs of infection require prompt medical assessment.
Care is required in patients with epilepsy, hepatic or renal impairment, cardiovascular disease, hypotension, or diabetes.
Hyponatraemia/SIADH, akathisia, QT prolongation, and ventricular arrhythmias have also been reported.
Treatment should generally be tapered gradually rather than discontinued abruptly. (Medicines.org.uk)
Pregnancy: Use should be based on an individual benefit-risk assessment. Neonatal monitoring may be appropriate when treatment continues into late pregnancy.
Breastfeeding: Mirtazapine is excreted into human milk in small amounts. The decision to continue breastfeeding or treatment should be individualised. (Medicines.org.uk)
Common adverse reactions include somnolence or sedation, increased appetite, weight gain, dry mouth, dizziness, and fatigue.
Other effects may include headache, abnormal dreams, tremor, orthostatic hypotension, constipation, and peripheral oedema.
Clinically important reactions include agranulocytosis, neutropenia, serotonin syndrome, hyponatraemia, seizures, mania/hypomania, QT prolongation, and severe cutaneous adverse reactions. (Medicines.org.uk)
Overdose may cause central nervous system depression, disorientation, prolonged sedation, tachycardia, and blood-pressure abnormalities.
Severe toxicity, including cardiac arrhythmias, may occur following very large or mixed overdoses.
There is no specific antidote. Management consists of supportive and symptomatic treatment, with ECG and vital-sign monitoring where clinically appropriate.
Mirtazapine must not be used concomitantly with MAO inhibitors.
Caution is required with serotonergic medicines including SSRIs, SNRIs, tramadol, triptans, lithium, linezolid, methylene blue, and St. John’s Wort because of the risk of serotonin syndrome.
Alcohol, benzodiazepines, opioids, sedating antihistamines, and other CNS depressants may increase sedation.
Carbamazepine and phenytoin may reduce Mirtazapine exposure, whereas strong CYP3A4 inhibitors may increase concentrations.
Caution is also appropriate with medicines that prolong the QT interval and with warfarin where INR monitoring may be clinically appropriate. (Medicines.org.uk)
Mirtazapine 15 mg per orodispersible tablet
REMERON SOLTAB 15 MG is registered in Thailand as an orodispersible tablet containing Mirtazapine 15 mg per tablet. The Thai FDA lists Organon (Thailand) Ltd. as the licence holder. (ข้อมูลยาแห่งชาติ)

The current REMERON/REMERONSolTab prescribing information recommends:
15 mg once daily as the starting dose

The usual effective dose range in current European product information is 15–45 mg/day. (Medicines.org.uk)
Clinical effects generally begin within 1–2 weeks, with a positive response to an adequate dose expected within approximately 2–4 weeks.
Patients who respond should generally remain on treatment for a sufficient period, usually at least 6 months, to maintain remission. (Medicines.org.uk)
For REMERON SOLTAB, remove the tablet from the blister only when ready to administer, place it on the tongue, allow it to disintegrate, and swallow with saliva. Water is generally not required. (DailyMed)
REMERON/REMERONSolTab carries a Boxed Warning regarding an increased risk of suicidal thoughts and behaviours in pediatric and young adult patients treated with antidepressants. Patients should be monitored closely for clinical worsening and emerging suicidal thoughts, particularly during early treatment and after dose changes. REMERON/REMERONSolTab is not approved for pediatric patients in the U.S. labeling. (Organon)
Fever, sore throat, oral ulcers, or other signs of infection require prompt assessment because they may indicate neutropenia or agranulocytosis.
Symptoms compatible with serotonin syndrome require urgent medical assessment.
Mirtazapine should generally be tapered gradually rather than stopped abruptly. (DailyMed)
The public Thai FDA record confirms the formulation and strength of REMERON SOLTAB 15 MG but does not display product-specific storage conditions on the retrieved public page. The Thai marketed product label should therefore take precedence. (ข้อมูลยาแห่งชาติ)
A current SmPC for Mirtazapine 15 mg orodispersible tablets states that no special storage conditions are required. REMERONSolTab should be removed from its blister only when ready for administration. (Medicines.org.uk)
