Levodopa 250 mg + Carbidopa 25 mg is an antiparkinsonian dopaminergic combination used to treat Parkinson’s disease and Parkinsonian syndromes. Levodopa is converted to dopamine in the brain, while Carbidopa inhibits peripheral Levodopa decarboxylation, allowing more Levodopa to reach the CNS and reducing peripheral adverse effects. Mechanism of Action

Levodopa is a metabolic precursor of dopamine that crosses the blood-brain barrier and is converted to dopamine within the CNS.

Carbidopa is a peripheral aromatic L-amino acid decarboxylase inhibitor that reduces peripheral conversion of Levodopa to dopamine without substantially entering the CNS.

The combination increases the amount of Levodopa reaching the brain and reduces peripheral adverse effects such as nausea and vomiting. DailyMed

Carbidopa and Levodopa have no antibacterial, antiviral, or antifungal activity.

Pharmacokinetics

Levodopa is absorbed from the gastrointestinal tract and competes with large neutral amino acids for intestinal and blood-brain barrier transport.

High-protein meals may therefore delay or reduce clinical response in some patients.

Carbidopa reduces peripheral Levodopa metabolism and increases the availability of Levodopa for CNS penetration.

Iron salts may reduce Carbidopa/Levodopa bioavailability through chelation. DailyMed

Contraindications

SINEMET is contraindicated in patients with hypersensitivity to any component.

Nonselective MAO inhibitors are contraindicated and should be discontinued at least 2 weeks before SINEMET.

Precautions

SINEMET may cause dyskinesia and involuntary movements, requiring individualized dose adjustment.

Hallucinations, psychotic symptoms, confusion, depression, and impulse-control disorders may occur.

Abrupt discontinuation may precipitate an NMS-like syndrome with hyperpyrexia, rigidity, altered consciousness, and autonomic dysfunction.

Use During Pregnancy and Breastfeeding

Pregnancy: Human data remain limited. Levodopa crosses the placenta. Treatment should be used only after an individual benefit-risk assessment.

Adverse Effects

The most prominent adverse effects include dyskinesia and nausea.

Other reactions include orthostatic hypotension, dizziness, palpitations, gastrointestinal symptoms, somnolence, insomnia, hallucinations, confusion, depression, and motor fluctuations.

Saliva, urine, or sweat may occasionally develop a red, brown, or black discoloration, which is generally benign. DailyMed

Overdose

There is no specific antidote.

Management is supportive and symptomatic, with airway management, hydration, and ECG monitoring where appropriate.

Drug Interactions

Nonselective MAO inhibitors: Contraindicated.

Antihypertensive agents: May increase symptomatic orthostatic hypotension.
