ข้อบ่งใช้ของ Imiquimod 5% cream ตาม SmPC ได้แก่ external genital and perianal warts (condylomata acuminata) ในผู้ใหญ่, small superficial basal cell carcinoma (sBCC) ในผู้ใหญ่ และ clinically typical non-hyperkeratotic, non-hypertrophic actinic keratoses บริเวณใบหน้าหรือหนังศีรษะในผู้ใหญ่ที่มีภูมิคุ้มกันปกติ เมื่อวิธีรักษาอื่นไม่เหมาะสม. Imiquimod 5% cream is a topical immune response modifier used for external genital and perianal warts, superficial basal cell carcinoma, and selected actinic keratoses. Its therapeutic effect is mediated primarily through local immune activation rather than direct antiviral activity. (Medicines.org.uk)
Imiquimod acts principally as a Toll-like receptor 7 (TLR7) agonist, stimulating innate and cell-mediated immune responses and inducing cytokines including interferon-α, tumour necrosis factor-α, and interleukins.
In genital warts, it therefore promotes immune-mediated clearance of HPV-associated lesions rather than directly inhibiting HPV replication.

Imiquimod is not an antibacterial, antifungal, or directly acting antiviral drug. Its effect on HPV-associated lesions results from immune modulation.

Approved indications include external genital and perianal warts in adults, small superficial basal cell carcinoma in adults, and selected clinically typical, non-hyperkeratotic, non-hypertrophic actinic keratoses of the face or scalp in immunocompetent adults. (Medicines.org.uk)
For external genital and perianal warts, apply a thin layer three times weekly before bedtime, leave on for 6–10 hours, then wash off with mild soap and water. Continue until visible warts have cleared or for a maximum of 16 weeks per episode. (Medicines.org.uk)
For superficial basal cell carcinoma, apply five times weekly for 6 weeks, generally before bedtime, leaving the cream on for approximately 8 hours.
