(Medicines.org.uk)
HMG-CoA reductase
(Medicines.org.uk)
(Medicines.org.uk)
Atorvastatin. (Medicines.org.uk)
(Medicines.org.uk)
(Medicines.org.uk)
(Medicines.org.uk)
(Medicines.org.uk)
(Medicines.org.uk)
ไม่มี specific antidote สำหรับ Atorvastatin overdose
(Medicines.org.uk)
Glecaprevir/pibrentasvir เป็นข้อห้ามใช้ร่วมกับ Lipitor
(Medicines.org.uk)
(Medicines.org.uk)
(Medicines.org.uk)

Atorvastatin 40 mg is an HMG-CoA reductase inhibitor (statin) used as an adjunct to diet to reduce LDL-C, total cholesterol, ApoB, and triglycerides and to help reduce cardiovascular risk in appropriately selected patients. The Thai FDA lists LIPITOR (TABLETS 40 MG) as a film-coated tablet containing Atorvastatin 40 mg per tablet, licensed to Viatris (Thailand) Ltd.
Atorvastatin is a selective competitive inhibitor of HMG-CoA reductase, the rate-limiting enzyme in hepatic cholesterol synthesis.
Reduced hepatic cholesterol increases LDL receptor expression, enhancing receptor-mediated uptake and clearance of circulating LDL-C. Atorvastatin also decreases VLDL production and consequently reduces LDL formation. (Medicines.org.uk)
None.
Atorvastatin has no antibacterial, antiviral, or antifungal activity.
Atorvastatin is rapidly absorbed after oral administration, with peak plasma concentrations generally occurring within approximately 1–2 hours.
It undergoes extensive hepatic metabolism predominantly through CYP3A4, producing pharmacologically active metabolites.
Atorvastatin is more than 98% protein bound. Its plasma elimination half-life is approximately 14 hours, while pharmacological activity persists longer because of active metabolites.
Elimination occurs predominantly through the bile following hepatic and extrahepatic metabolism. (Medicines.org.uk)
Contraindicated in patients with hypersensitivity to Atorvastatin or any excipient.
Contraindicated in patients with active liver disease or unexplained persistent serum transaminase elevations exceeding three times the upper limit of normal.
The current Lipitor SmPC also contraindicates treatment during pregnancy, breastfeeding, and in women of childbearing potential not using appropriate contraception, and during concomitant use of glecaprevir/pibrentasvir. (Medicines.org.uk)
Atorvastatin may cause myalgia, myositis, myopathy, and rhabdomyolysis. Patients should promptly report unexplained muscle pain, cramps, tenderness, or weakness, particularly when accompanied by fever or malaise. (Medicines.org.uk)
Risk is increased in patients with renal impairment, hypothyroidism, a history of muscle disorders or previous statin/fibrate toxicity, substantial alcohol intake, or exposure to interacting medicines.
Liver function should be assessed before treatment and subsequently when clinically indicated.
Rare immune-mediated necrotizing myopathy may persist despite statin discontinuation and may require specialist treatment.
Statins have also rarely been associated with new-onset or worsening myasthenia gravis or ocular myasthenia. (Medicines.org.uk)
Pregnancy: Lipitor is contraindicated during pregnancy according to the referenced SmPC. Treatment should be discontinued when pregnancy occurs. (Medicines.org.uk)
Breastfeeding: Lipitor is contraindicated during breastfeeding according to the same product information. (Medicines.org.uk)
Adverse effects may include headache, myalgia, arthralgia, nausea, diarrhoea, constipation, flatulence, dyspepsia, and other gastrointestinal symptoms.
Laboratory abnormalities may include elevations in hepatic transaminases or creatine kinase.
Clinically important reactions include myopathy, rhabdomyolysis, hepatitis, pancreatitis, hypersensitivity reactions, and immune-mediated necrotizing myopathy. (Medicines.org.uk)
There is no specific antidote for Atorvastatin overdose.
Management consists of supportive and symptomatic treatment, with appropriate monitoring of hepatic, renal, muscular, and cardiovascular status.
Haemodialysis is unlikely to substantially increase elimination because Atorvastatin is highly protein bound.
Atorvastatin is metabolised primarily through CYP3A4 and is also a substrate of hepatic transport proteins. Medicines that increase Atorvastatin exposure may substantially increase the risk of myopathy or rhabdomyolysis. (Medicines.org.uk)
Strong CYP3A4 inhibitors include clarithromycin, telithromycin, ketoconazole, voriconazole, itraconazole, posaconazole, and certain antiviral agents.
Glecaprevir/pibrentasvir is contraindicated.
Gemfibrozil, other fibrates, ezetimibe, niacin, erythromycin, and certain HCV antiviral medicines may increase the risk of muscle toxicity. (Medicines.org.uk)
Atorvastatin should not be co-administered with systemic fusidic acid or within 7 days after fusidic acid treatment.
When administered with elbasvir/grazoprevir or letermovir, the current Lipitor SmPC limits Atorvastatin to 20 mg/day. (Medicines.org.uk)
Atorvastatin 40 mg per film-coated tablet
The Thai FDA identifies LIPITOR (TABLETS 40 MG) as a film-coated tablet containing Atorvastatin 40 mg per tablet, licensed to Viatris (Thailand) Ltd.
The Viatris Lipitor SmPC states that each tablet contains Atorvastatin 40 mg as atorvastatin calcium trihydrate. The reference tablet is a white, round, approximately 9.5-mm film-coated tablet marked “40” on one side and “ATV” on the other. (Medicines.org.uk)
Specified pack size: 3 × 10 tablets = 30 tablets
Lipitor is indicated as an adjunct to diet for reduction of elevated total cholesterol, LDL-C, ApoB, and triglycerides in adults, adolescents, and children aged 10 years or older with primary hypercholesterolaemia, including heterozygous familial hypercholesterolaemia, or combined/mixed hyperlipidaemia when diet and other non-pharmacological measures are inadequate. (Medicines.org.uk)
It is also indicated in adults with homozygous familial hypercholesterolaemia as an adjunct to other lipid-lowering treatment or when such treatments are unavailable.
Lipitor is additionally indicated for prevention of cardiovascular events in adults at high risk of a first cardiovascular event, together with management of other modifiable risk factors. (Medicines.org.uk)
Patients should follow an appropriate cholesterol-lowering diet before and throughout treatment.
The usual starting dose is:
Atorvastatin 10 mg once daily
The general dosage range is:
10–80 mg once daily
Dose selection should be individualised according to baseline LDL-C, treatment goals, and response. Dose adjustments should generally be made at intervals of 4 weeks or longer, with a maximum recommended dose of 80 mg once daily. (Medicines.org.uk)
For heterozygous familial hypercholesterolaemia, treatment generally begins at 10 mg/day and may be titrated to 40 mg/day, with further escalation when clinically appropriate.
Lipitor may be administered once daily at any time of day, with or without food. (Medicines.org.uk)
LIPITOR 40 mg is above the usual initial dose and should generally be used after assessment of LDL-C targets, cardiovascular risk, treatment response, and tolerability.
Patients should promptly report unexplained muscle pain, cramps, tenderness, or weakness, particularly when associated with fever, marked fatigue, or dark urine, because myopathy or rhabdomyolysis may occur. (Medicines.org.uk)
Patients with renal impairment, hypothyroidism, a history of muscle disorders, advanced age, or interacting medicines may have an increased risk of muscle toxicity.
Symptoms suggestive of hepatic injury, including jaundice, dark urine, loss of appetite, or unusual fatigue, require medical assessment.
Concomitant medicines should be reviewed carefully, particularly azole antifungals, macrolide antibiotics, antiviral agents, ciclosporin, fibrates, daptomycin, and fusidic acid. (Medicines.org.uk)
The current Viatris Lipitor 40 mg SmPC states that no special storage conditions are required. (Medicines.org.uk)
For Lipitor supplied in Thailand, the Thai marketed package label and local product information should take precedence if they provide different instructions.
