Mirtazapine 30 mg is an antidepressant indicated for the treatment of major depressive disorder / major depressive episodes in adults. It enhances central noradrenergic and serotonergic neurotransmission and has clinically relevant H1-histamine receptor antagonistic activity, which contributes to its sedative effects and may increase appetite. The Thai-registered REMERON SOLTAB 30 MG is a 30 mg orodispersible tablet licensed to Organon (Thailand) Ltd. (ข้อมูลยาแห่งชาติ)
Mirtazapine is a centrally active presynaptic α2-adrenergic antagonist that increases central noradrenergic and serotonergic neurotransmission.
It blocks 5-HT2 and 5-HT3 receptors, favouring serotonergic neurotransmission through 5-HT1 receptors, and also exhibits significant H1-histamine receptor antagonism associated with sedation. (Medicines.org.uk)
None.
Mirtazapine has no antibacterial, antiviral, or antifungal activity.
Mirtazapine is rapidly and well absorbed after oral administration, with an oral bioavailability of approximately 50%. Peak plasma concentrations are reached after approximately 2 hours, and food does not materially affect its pharmacokinetics. (Medicines.org.uk)
Plasma protein binding is approximately 85%. Metabolism occurs through demethylation and oxidation, involving CYP1A2, CYP2D6, and CYP3A4.
The mean elimination half-life is approximately 20–40 hours, and steady state is generally reached within about 3–4 days. (Medicines.org.uk)
Contraindicated in patients with hypersensitivity to Mirtazapine or any excipient.
Concomitant treatment with MAO inhibitors, or use within 14 days after stopping an MAOI, is contraindicated. An MAOI should likewise not be started until at least 14 days after Mirtazapine is discontinued. (Organon)
Patients should be monitored for clinical worsening, suicidal thoughts, and suicidal behaviour, particularly during early treatment and after dose changes. (Organon)
Mirtazapine may cause clinically significant somnolence, dizziness, and impaired alertness.
Rare neutropenia and agranulocytosis have been reported. Fever, sore throat, stomatitis, or other signs of infection require prompt medical assessment. (Organon)
Care is required in patients with epilepsy, hepatic impairment, renal impairment, cardiovascular disease, hypotension, or diabetes.
Hyponatraemia/SIADH, akathisia, QT prolongation, and ventricular arrhythmias have also been reported.
Treatment should generally be tapered gradually rather than discontinued abruptly. (Medicines.org.uk)
Pregnancy: Treatment should be used when the expected clinical benefit justifies the potential risk. Neonatal observation is appropriate when exposure continues into late pregnancy.
Breastfeeding: Mirtazapine is excreted into human milk in small amounts. The decision to continue breastfeeding or treatment should be individualised. (Medicines.org.uk)
Common adverse reactions include somnolence or sedation, increased appetite, weight gain, dry mouth, dizziness, and fatigue.
Other effects may include headache, abnormal dreams, tremor, orthostatic hypotension, constipation, and peripheral oedema.
Clinically important reactions include agranulocytosis, neutropenia, serotonin syndrome, hyponatraemia, seizures, mania/hypomania, QT prolongation, and severe cutaneous adverse reactions. (Medicines.org.uk)
Overdose may cause central nervous system depression, disorientation, prolonged sedation, tachycardia, and blood-pressure abnormalities.
Severe toxicity, including cardiac arrhythmias, may occur following very large or mixed overdoses.
There is no specific antidote. Treatment is supportive and symptomatic, with monitoring of vital signs and ECG when clinically indicated. (Organon)
Mirtazapine must not be used concomitantly with MAO inhibitors.
Caution is required with serotonergic medicines including SSRIs, SNRIs, tramadol, triptans, lithium, linezolid, methylene blue, and St. John’s Wort because of the risk of serotonin syndrome.
Alcohol, benzodiazepines, opioids, sedating antihistamines, and other CNS depressants may increase sedation.
Carbamazepine and phenytoin can reduce Mirtazapine exposure, whereas potent CYP3A4 inhibitors may increase concentrations.
Caution is also warranted with medicines associated with QT prolongation and with warfarin where INR monitoring may be appropriate. (Medicines.org.uk)
Mirtazapine 30 mg per orodispersible tablet
REMERON SOLTAB 30 MG is registered in Thailand as an orodispersible tablet containing Mirtazapine 30 mg per tablet. The Thai FDA lists Organon (Thailand) Ltd. as the licence holder. (ข้อมูลยาแห่งชาติ)
Pack size specified: 30 tablets
Indicated in adults for the treatment of major depressive disorder / episodes of major depression. (Medicines.org.uk)
The U.S. REMERON/REMERONSolTab prescribing information recommends a starting dose of:
15 mg once daily
The usual effective dose range is 15–45 mg/day, with a maximum recommended dose of 45 mg/day. European SmPCs also permit a starting dose of 15 or 30 mg according to clinical judgement. (Medicines.org.uk)
Mirtazapine is generally administered once daily, preferably in the evening before sleep.
Clinical improvement generally begins within 1–2 weeks, and an adequate dose should produce a positive response within approximately 2–4 weeks.
Patients who respond should generally continue treatment for a sufficient period, commonly at least 6 months, to maintain remission. (Medicines.org.uk)
For REMERON SOLTAB, open the blister only when ready to take the medicine, place the tablet on the tongue, allow it to disintegrate, and swallow with saliva. Water is generally not required. (Organon)
REMERON/REMERONSolTab carries a Boxed Warning regarding an increased risk of suicidal thoughts and behaviours in pediatric and young adult patients treated with antidepressants. Patients should be monitored closely for clinical worsening or emerging suicidal thoughts, particularly during early treatment and after dose adjustments. (Organon)
Fever, sore throat, oral ulcers, or other signs of infection require prompt assessment because they may indicate neutropenia or agranulocytosis. (Organon)
Symptoms compatible with serotonin syndrome require urgent medical assessment.
Mirtazapine should generally be tapered gradually rather than stopped abruptly. (Medicines.org.uk)
The public Thai FDA record confirms the formulation and strength of REMERON SOLTAB 30 MG but does not display product-specific storage conditions on the retrieved public page. The Thai marketed label should therefore take precedence. (ข้อมูลยาแห่งชาติ)
Because this is an orodispersible tablet, it should be removed from the blister only when ready for administration. A current SmPC for Mirtazapine 30 mg orodispersible tablets states that no special storage conditions are required. (Medicines.org.uk)
