การออกฤทธิ์

Levodopa เป็น precursor ของ dopamine ที่สามารถผ่าน blood-brain barrier แล้วถูกเปลี่ยนเป็น dopamine ในสมอง ช่วยทดแทน dopamine ที่ลดลงในผู้ป่วย Parkinson’s disease

เภสัชจลนศาสตร์

Levodopa ถูกดูดซึมจากทางเดินอาหารและแข่งขันกับ large neutral amino acids ทั้งในการดูดซึมผ่านลำไส้และการเข้าสู่สมอง ดังนั้นอาหารโปรตีนสูงอาจทำให้การดูดซึมและ clinical response ลดลงในผู้ป่วยบางราย. DailyMed

ข้อห้ามใช้

ห้ามใช้ในผู้ที่แพ้ Carbidopa, Levodopa หรือส่วนประกอบของยา

ห้ามใช้ร่วมกับ nonselective monoamine oxidase inhibitors (MAOIs) และต้องหยุด MAOI กลุ่มนี้อย่างน้อย 2 สัปดาห์ก่อนเริ่ม SINEMET โดย selective MAO-B inhibitor เช่น selegiline อาจใช้ร่วมกันได้ตามขนาดที่ผู้ผลิตแนะนำภายใต้การติดตามอย่างเหมาะสม. DailyMed

ข้อความระวัง

SINEMET อาจทำให้เกิด dyskinesia หรือ involuntary movements และเนื่องจาก Carbidopa ทำให้ Levodopa เข้าสู่สมองได้มากขึ้น อาการเหล่านี้อาจเกิดที่ขนาดต่ำกว่าการใช้ Levodopa เดี่ยว จึงต้องปรับขนาดตามอาการของผู้ป่วย. DailyMed

อาจเกิด orthostatic hypotension, dizziness และ syncope โดยเฉพาะเมื่อเริ่มยา ปรับขนาด หรือใช้ร่วมกับยาลดความดัน

มีรายงาน somnolence และ sudden sleep onset ระหว่างทำกิจวัตรประจำวัน รวมถึงขณะขับรถ โดยบางรายไม่มีอาการง่วงเตือนมาก่อน หากเคยเกิดอาการหลับกะทันหันไม่ควรขับรถหรือใช้เครื่องจักร. DailyMed

อาจเกิด hallucinations, psychosis, confusion, depression และ impulse-control disorders เช่น pathological gambling, hypersexuality หรือ uncontrolled spending ผู้ดูแลควรสังเกตการเปลี่ยนแปลงด้านพฤติกรรม. DailyMed

ไม่ควรหยุดยาอย่างรวดเร็ว เนื่องจากมีรายงาน hyperpyrexia, rigidity และ confusion คล้าย neuroleptic malignant syndrome (NMS) หลังลดขนาดหรือหยุดยาอย่างกะทันหัน. DailyMed

Current U.S. label ฉบับปี 2026 เพิ่มคำเตือนเรื่อง Vitamin B6 deficiency โดยเฉพาะเมื่อใช้ขนาดสูงหรือใช้เป็นเวลานาน อาการอาจรวม depression, confusion, cheilosis, glossitis, dermatitis, anemia, neuropathy และในรายรุนแรงอาจเกิด seizures จึงระบุให้ประเมิน Vitamin B6 ก่อนเริ่มยาและติดตามระหว่างการรักษาตามความเหมาะสม. อาการข้างเคียง

สำหรับผู้ป่วยที่เริ่ม Carbidopa/Levodopa ใหม่ Current Thai MIMS และ current U.S. prescribing information ระบุขนาดเริ่มต้นที่ใช้บ่อยสำหรับ SINEMET 25/100 คือ:

1 เม็ด วันละ 3 ครั้ง

ซึ่งให้ Carbidopa รวม 75 mg/day. MIMS

สามารถปรับเพิ่ม ครั้งละ 1 เม็ดทุกวันหรือวันเว้นวัน ตามการตอบสนอง จนถึงประมาณ 8 เม็ดของ SINEMET 25/100 ต่อวันใน regimen ที่เหมาะสม. DailyMed

หากเกิด การเคลื่อนไหวผิดปกติมากขึ้น กระตุก บิดเกร็ง หรือ dyskinesia ควรแจ้งแพทย์ เพราะอาจจำเป็นต้องปรับขนาดยา

หากเกิด ง่วงมากหรือหลับกะทันหันขณะทำกิจกรรม ไม่ควรขับรถ ใช้เครื่องจักร หรือทำงานที่อาจเกิดอันตรายจนกว่าจะได้รับการประเมิน. DailyMed

หากเกิด hallucinations, confusion, พฤติกรรมผิดปกติ หรือมีแรงกระตุ้นที่ควบคุมไม่ได้ เช่น พนัน ใช้เงิน หรือความต้องการทางเพศเพิ่มขึ้นผิดปกติ ควรแจ้งแพทย์หรือผู้ดูแล. DailyMed

Carbidopa 25 mg + Levodopa 100 mg is an antiparkinsonian dopaminergic combination used to treat Parkinson’s disease and Parkinsonian syndromes. Levodopa is converted to dopamine within the brain, while Carbidopa inhibits peripheral Levodopa decarboxylation, increasing the amount reaching the CNS and reducing peripheral adverse effects such as nausea and vomiting. The Thai FDA identifies SINEMET as a tablet containing Levodopa 100 mg + Carbidopa 25 mg, manufactured by M & H Manufacturing Co., Ltd., with Thai MiT/MIMS sources confirming a 10 × 10 tablet pack. ข้อมูลยาแห่งชาติ

Mechanism of Action

Levodopa is a metabolic precursor of dopamine that crosses the blood-brain barrier and is converted to dopamine within the CNS, thereby replacing deficient striatal dopamine in Parkinson’s disease.

When Levodopa is administered alone, a substantial proportion is converted to dopamine in peripheral tissues before reaching the brain.

Carbidopa is a peripheral aromatic L-amino acid decarboxylase inhibitor that substantially reduces peripheral Levodopa decarboxylation without significantly crossing the blood-brain barrier.

This allows a greater proportion of Levodopa to reach the brain, permits lower Levodopa doses, and reduces peripheral nausea and vomiting. DailyMed

Antimicrobial Spectrum

None.

Carbidopa and Levodopa have no antibacterial, antiviral, or antifungal activity.

Pharmacokinetics

Levodopa is absorbed from the gastrointestinal tract and competes with large neutral amino acids for intestinal transport and transport into the CNS.

High-protein meals may therefore reduce or delay Levodopa absorption and clinical response in some patients. DailyMed

Carbidopa inhibits peripheral Levodopa metabolism, increasing circulating Levodopa available for CNS penetration.

Iron salts can chelate Carbidopa/Levodopa and reduce bioavailability. DailyMed

Contraindications

SINEMET is contraindicated in patients with hypersensitivity to any component.

Nonselective MAO inhibitors are contraindicated and must be discontinued at least 2 weeks before SINEMET is initiated. Selective MAO-B inhibitors may be used at recommended doses with appropriate monitoring. DailyMed

SINEMET is also contraindicated in patients with narrow-angle glaucoma under current U.S. labeling. DailyMed

Precautions

SINEMET may cause dyskinesias and other involuntary movements, sometimes at lower Levodopa doses than with Levodopa alone. DailyMed

Orthostatic hypotension, dizziness, and syncope may occur, especially during initiation or titration.

Patients may experience somnolence or sudden sleep onset during daily activities, occasionally without warning. Patients who experience these events should not drive or operate machinery. DailyMed

Hallucinations, psychotic symptoms, confusion, depression, and impulse-control disorders including pathological gambling, hypersexuality, and compulsive spending have been reported. DailyMed

Abrupt dose reduction or discontinuation may precipitate hyperpyrexia, rigidity, confusion, and an NMS-like syndrome. DailyMed

The current 2026 U.S. label also warns that chronic Carbidopa/Levodopa therapy may contribute to Vitamin B6 deficiency, potentially producing neuropsychiatric symptoms, dermatitis, anaemia, neuropathy, or seizures. Baseline and periodic Vitamin B6 assessment is now recommended. DailyMed

Patients with Parkinson’s disease have an increased epidemiological risk of melanoma; regular skin monitoring is advised. DailyMed

Use During Pregnancy and Breastfeeding

Pregnancy: Adequate controlled studies in pregnant women are lacking. Levodopa crosses the human placenta. SINEMET should therefore be used only after careful assessment of anticipated benefit versus potential maternal and fetal risk. DailyMed

Breastfeeding: Levodopa has been detected in human milk. Caution is advised when SINEMET is administered to a breastfeeding woman. DailyMed

Adverse Effects

The most commonly reported adverse effects are dyskinesias and nausea. DailyMed

Other reactions include orthostatic hypotension, dizziness, palpitations, vomiting, diarrhoea, constipation, dry mouth, somnolence, sleep disturbances, hallucinations, confusion, depression, and “on-off” motor fluctuations.

Impulse-control disorders and haematological abnormalities such as leukopenia, thrombocytopenia, and haemolytic anaemia have also been reported.

Saliva, urine, or sweat may occasionally develop a red, brown, or black discoloration, which is generally clinically insignificant. DailyMed

Overdose

There is no specific antidote.

Management is supportive and symptomatic, with airway support, appropriate fluid therapy, and ECG monitoring for arrhythmias.

The value of dialysis in SINEMET overdose has not been established. DailyMed

Drug Interactions

Nonselective MAO inhibitors: Contraindicated; discontinue at least 2 weeks before SINEMET. DailyMed

Antihypertensive medicines: May increase symptomatic postural hypotension.

Tricyclic antidepressants: Rare hypertension and dyskinesia have been reported.

Dopamine D2 antagonists including phenothiazines, butyrophenones, and risperidone, as well as isoniazid, may reduce Levodopa efficacy.

Phenytoin and papaverine: May reduce the therapeutic response.

Reserpine and tetrabenazine: Concomitant use is not recommended because of monoamine depletion.

Iron salts: May reduce Carbidopa/Levodopa bioavailability through chelation. DailyMed

Indications

SINEMET is indicated for treatment of Parkinson’s disease and Parkinsonian syndromes, particularly Levodopa-responsive motor symptoms such as bradykinesia, rigidity, and tremor. DailyMed

Carbidopa reduces peripheral Levodopa metabolism, allowing lower Levodopa doses and reducing peripheral nausea and vomiting compared with Levodopa alone. DailyMed

Recommended Dosage

SINEMET dosing must be carefully individualized and titrated according to therapeutic response and adverse effects. MIMS

For patients initiating Carbidopa/Levodopa therapy, a commonly referenced starting regimen for SINEMET 25/100 is:

1 tablet three times daily

The dose may be increased by 1 tablet each day or every other day as clinically required, up to approximately 8 tablets of SINEMET 25/100 daily under appropriate medical supervision. DailyMed

Peripheral dopa decarboxylase is generally adequately inhibited with approximately 70–100 mg of Carbidopa daily. Lower Carbidopa exposure is associated with a greater likelihood of nausea and vomiting. DailyMed

Patients transferring from Levodopa alone should discontinue Levodopa for at least 12 hours before starting SINEMET, with the new regimen selected by the prescriber. DailyMed

SINEMET should not be abruptly discontinued without medical supervision.

Warnings in Accordance with Public Safety Announcements

Report worsening involuntary movements or dyskinesia, as dose adjustment may be required.

Patients who develop excessive daytime sleepiness or sudden sleep episodes should not drive or operate dangerous machinery. DailyMed

New hallucinations, confusion, compulsive gambling, uncontrolled spending, or markedly increased sexual urges should be reported to the prescriber. DailyMed

Abrupt withdrawal should be avoided because of the risk of an NMS-like syndrome with hyperthermia, rigidity, altered consciousness, and autonomic instability. DailyMed
