(Product Details)

SANDOZ ATORVASTATIN GMBH 20MG 3×10 Tablets

(Medicines.org.uk)

HMG-CoA reductase

(Medicines.org.uk)

(Medicines.org.uk)

(Medicines.org.uk)

SANDOZ ATORVASTATIN GMBH 20MG 3×10 Tablets

Atorvastatin 20 mg is an HMG-CoA reductase inhibitor (statin) used as an adjunct to diet to reduce LDL-C, total cholesterol, ApoB, and triglycerides and to reduce cardiovascular risk in appropriately selected patients. The Thai-registered ATORVASTATIN SANDOZ GMBH (20MG) is a 20 mg film-coated tablet, manufactured by LEK S.A. in Poland and licensed to Sandoz (Thailand) Ltd.

Mechanism of Action

Atorvastatin is a selective competitive inhibitor of HMG-CoA reductase, the rate-limiting enzyme in hepatic cholesterol biosynthesis.

Reduced hepatic cholesterol increases LDL receptor expression, enhancing receptor-mediated uptake and clearance of circulating LDL-C. Atorvastatin also reduces VLDL production and consequently lowers LDL production. (Medicines.org.uk)

Antimicrobial Spectrum

None.

Atorvastatin has no antibacterial, antiviral, or antifungal activity.

Pharmacokinetics

Atorvastatin is rapidly absorbed after oral administration, with peak plasma concentrations generally occurring within approximately 1–2 hours.

It undergoes extensive hepatic metabolism, predominantly through CYP3A4, producing pharmacologically active metabolites.

Atorvastatin is more than 98% protein bound. Its plasma elimination half-life is approximately 14 hours, while pharmacological HMG-CoA reductase inhibitory activity persists longer because of active metabolites.

Elimination occurs mainly through the bile following hepatic and extrahepatic metabolism.

Contraindications

Contraindicated in patients with hypersensitivity to Atorvastatin or any excipient.

Contraindicated in patients with active liver disease or unexplained persistent elevations of serum transaminases exceeding three times the upper limit of normal. (Medicines.org.uk)

The referenced Sandoz SmPC also contraindicates use during pregnancy, breastfeeding, and in women of childbearing potential not using appropriate contraception, as well as concomitant therapy with glecaprevir/pibrentasvir. (Medicines.org.uk)

Precautions

Atorvastatin may cause myalgia, myositis, myopathy, and rhabdomyolysis. Patients should promptly report unexplained muscle pain, cramps, tenderness, or weakness, particularly when accompanied by fever or malaise. (Medicines.org.uk)

Risk factors include renal impairment, hypothyroidism, a history of hereditary or statin-associated muscle disorders, liver disease, substantial alcohol intake, advanced age, and interacting medicines.

Liver function tests should be performed before treatment and thereafter when clinically indicated.

Rare immune-mediated necrotizing myopathy may persist despite discontinuation and may require specialist treatment.

Statins have also rarely been reported to induce or worsen myasthenia gravis or ocular myasthenia. (Medicines.org.uk)

Blood glucose may rise modestly in susceptible patients.

Use During Pregnancy and Breastfeeding

Pregnancy: Atorvastatin is contraindicated during pregnancy according to the referenced Sandoz SmPC. (Medicines.org.uk)

Breastfeeding: Atorvastatin is contraindicated during breastfeeding according to the same product information. (Medicines.org.uk)

Adverse Effects

Adverse effects may include headache, myalgia, arthralgia, nausea, diarrhoea, constipation, dyspepsia, and other gastrointestinal symptoms.

Laboratory abnormalities may include increased hepatic transaminases or creatine kinase.

Clinically important reactions include myopathy, rhabdomyolysis, hepatic injury, immune-mediated necrotizing myopathy, hypersensitivity reactions, and rare interstitial lung disease. (Medicines.org.uk)

Overdose

There is no specific antidote for Atorvastatin overdose.

Management consists of supportive and symptomatic treatment, with appropriate monitoring of hepatic, renal, muscular, and cardiovascular status.

Haemodialysis is unlikely to substantially increase elimination because Atorvastatin is highly protein bound.

Drug Interactions

Atorvastatin is metabolised primarily through CYP3A4 and is also a substrate of hepatic transport proteins.

Potent CYP3A4 or transporter inhibitors such as clarithromycin, ketoconazole, voriconazole, itraconazole, posaconazole, ciclosporin, and certain HIV protease inhibitors may markedly increase Atorvastatin exposure and the risk of myopathy or rhabdomyolysis. (Medicines.org.uk)

Glecaprevir/pibrentasvir is contraindicated. (Medicines.org.uk)

Gemfibrozil, other fibrates, niacin, ezetimibe, erythromycin, and some hepatitis C antivirals may increase muscle toxicity risk.

Atorvastatin must not be administered concomitantly with systemic fusidic acid or within 7 days after stopping fusidic acid; statin therapy is generally suspended for the duration of systemic fusidic acid treatment. (Medicines.org.uk)

When used with elbasvir/grazoprevir or letermovir, the Atorvastatin dose should not exceed 20 mg/day according to the referenced Sandoz SmPC. (Medicines.org.uk)

Drug Name and Strength

Atorvastatin 20 mg per film-coated tablet

ATORVASTATIN SANDOZ GMBH (20MG) is registered in Thailand as a film-coated tablet containing Atorvastatin 20 mg per tablet. It is manufactured by LEK S.A., Republic of Poland, and the Thai FDA lists Sandoz (Thailand) Ltd. as the licence holder.

Current Sandoz UK product information confirms that each film-coated tablet contains 20 mg Atorvastatin as atorvastatin calcium trihydrate. The reference tablet is described as a white to off-white, round, biconvex tablet marked “A20” on one side, with a breaking notch on the other. (Medicines.org.uk)

Specified Thai pack size: 3 × 10 tablets = 30 tablets. (MIMS)

Indications

Atorvastatin is indicated as an adjunct to diet for reduction of elevated total cholesterol, LDL-C, ApoB, and triglycerides in adults, adolescents, and children aged 10 years or older with primary hypercholesterolaemia, including heterozygous familial hypercholesterolaemia, or combined/mixed hyperlipidaemia when diet and other non-pharmacological measures are inadequate. (Medicines.org.uk)

It is also indicated to reduce total cholesterol and LDL-C in adults with homozygous familial hypercholesterolaemia as an adjunct to other lipid-lowering therapy or when such therapy is unavailable. (Medicines.org.uk)

Atorvastatin is additionally indicated for prevention of cardiovascular events in adults considered at high risk of a first cardiovascular event, together with management of other modifiable risk factors. (Medicines.org.uk)

Recommended Dosage

Patients should follow an appropriate cholesterol-lowering diet before and throughout treatment.

The usual adult starting dose is:

Atorvastatin 10 mg once daily

The general dosage range is:

10–80 mg once daily

Dose selection should be individualised according to baseline LDL-C, treatment goals, and clinical response. Dose adjustments should generally be made at intervals of 4 weeks or longer, with a maximum recommended dose of 80 mg once daily. (Medicines.org.uk)

For primary hypercholesterolaemia and mixed hyperlipidaemia, many patients are controlled with 10 mg once daily; 20 mg once daily may be appropriate when greater LDL-C reduction is required.

For heterozygous familial hypercholesterolaemia, treatment generally begins at 10 mg/day and is titrated according to response.

For homozygous familial hypercholesterolaemia, the referenced dose range is 10–80 mg/day as adjunctive treatment. (Medicines.org.uk)

Atorvastatin may be administered once daily at any time of day, with or without food. (Medicines.org.uk)

No dosage adjustment is generally required in renal impairment. Hepatic impairment requires caution, and active liver disease is a contraindication. (Medicines.org.uk)

Warnings in Accordance with Public Safety Announcements

Patients should promptly report unexplained muscle pain, cramps, tenderness, or weakness, particularly when accompanied by fever, marked fatigue, or dark urine, because myopathy or rhabdomyolysis may occur. (Medicines.org.uk)

Patients with renal impairment, hypothyroidism, a history of muscle disorders, advanced age, or exposure to interacting medicines may have increased risk of muscle toxicity.

Symptoms suggestive of hepatic injury, including jaundice, dark urine, loss of appetite, or unusual fatigue, require medical assessment.

Concomitant medicines should be reviewed carefully, particularly azole antifungals, macrolide antibiotics, antiviral agents, ciclosporin, fibrates, daptomycin, and fusidic acid, because some combinations can substantially increase the risk of myopathy or rhabdomyolysis. (Medicines.org.uk)

Pregnancy and breastfeeding are contraindications according to the referenced Sandoz SmPC. (Medicines.org.uk)

Storage Conditions

The Thai FDA confirms that ATORVASTATIN SANDOZ GMBH 20 mg is a film-coated tablet, but the retrieved public Thai record does not clearly display product-specific storage instructions.

The Thai marketed product label and local package insert should therefore take precedence.

Storage instructions from other Sandoz markets may be useful as reference information but should not automatically be treated as specific to the Thai product unless confirmed on the Thai label.
