SANDOZ ATORVASTATIN GMBH 10MG 3×10’S
(Medicines.org.uk)
Atorvastatin overdose
การใช้ร่วมกับ fibrates, ciclosporin, colchicine หรือ lipid-modifying doses of niacin อาจเพิ่มความเสี่ยง muscle toxicity
Atorvastatin

Atorvastatin 10 mg is an HMG-CoA reductase inhibitor (statin) used as an adjunct to diet to reduce LDL-C, total cholesterol, ApoB, and triglycerides and to reduce cardiovascular risk in appropriately selected patients. It works primarily by reducing hepatic cholesterol synthesis and increasing hepatic clearance of circulating LDL. The Thai-registered ATORVASTATIN SANDOZ GMBH (10MG) is a 10 mg film-coated tablet, manufactured by LEK S.A. in Poland and licensed to Sandoz (Thailand) Ltd.
Atorvastatin is a selective competitive inhibitor of HMG-CoA reductase, the rate-limiting enzyme in hepatic cholesterol biosynthesis.
Reduced hepatic cholesterol increases LDL receptor expression, enhancing receptor-mediated uptake and clearance of circulating LDL-C. Atorvastatin also reduces VLDL production and consequently lowers LDL production. (Medicines.org.uk)
None.
Atorvastatin has no antibacterial, antiviral, or antifungal activity.
Atorvastatin is rapidly absorbed after oral administration, with peak plasma concentrations generally occurring within approximately 1–2 hours.
It undergoes extensive hepatic metabolism, predominantly through CYP3A4, producing pharmacologically active hydroxylated metabolites.
Atorvastatin is more than 98% protein bound. Its plasma elimination half-life is approximately 14 hours, while HMG-CoA reductase inhibitory activity persists longer because of active metabolites.
Elimination occurs predominantly through the bile following hepatic and extrahepatic metabolism.
Contraindicated in patients with hypersensitivity to Atorvastatin or any excipient.
Contraindicated in patients with active liver disease or unexplained persistent elevations of serum transaminases exceeding three times the upper limit of normal.
Pregnancy recommendations differ between regulatory jurisdictions; the locally approved Thai product information should take precedence.
Atorvastatin may cause myalgia, myopathy, and rhabdomyolysis. Patients should promptly report unexplained muscle pain, tenderness, or weakness, particularly when accompanied by fever or malaise.
The risk of muscle toxicity is increased by renal impairment, uncontrolled hypothyroidism, advanced age, and interacting medicines that raise Atorvastatin exposure.
Liver function should be assessed before treatment and subsequently when clinically indicated.
Rare immune-mediated necrotizing myopathy may persist despite statin discontinuation and may require specialist management.
Statins may cause modest increases in fasting glucose and HbA1c in susceptible patients.
Pregnancy: Atorvastatin is generally discontinued when pregnancy is recognised in most patients because treatment of hyperlipidaemia can usually be interrupted during pregnancy. Exceptional very-high-risk patients require individual specialist assessment.
Breastfeeding: Atorvastatin is generally not recommended during breastfeeding because a clinically relevant risk to the nursing infant cannot be excluded.
Adverse effects may include headache, myalgia, arthralgia, nausea, diarrhoea, constipation, dyspepsia, and other gastrointestinal symptoms.
Laboratory abnormalities may include elevations in hepatic transaminases or creatine kinase.
Clinically important reactions include myopathy, rhabdomyolysis, hepatic injury, immune-mediated necrotizing myopathy, and hypersensitivity reactions.
Small increases in fasting glucose or HbA1c may occur.
There is no specific antidote for Atorvastatin overdose.
Management consists of supportive and symptomatic treatment, including appropriate monitoring of hepatic, renal, muscular, and cardiovascular status.
Haemodialysis is unlikely to substantially increase elimination because Atorvastatin is highly protein bound.
Atorvastatin is metabolised predominantly by CYP3A4 and is also a substrate of hepatic transport proteins.
Strong CYP3A4 inhibitors such as clarithromycin, itraconazole, ketoconazole, posaconazole, and certain antiviral agents may substantially increase Atorvastatin exposure and the risk of myopathy or rhabdomyolysis.
Concomitant use with fibrates, ciclosporin, colchicine, or lipid-modifying doses of niacin may further increase muscle toxicity.
Atorvastatin should not generally be administered concomitantly with systemic fusidic acid; temporary interruption of statin therapy may be required.
Large quantities of grapefruit juice may increase Atorvastatin concentrations.
Dose restrictions may apply with certain antiviral or transporter-inhibiting medicines and should be checked before co-administration.
Atorvastatin 10 mg per film-coated tablet
ATORVASTATIN SANDOZ GMBH (10MG) is registered in Thailand as a film-coated tablet containing Atorvastatin 10 mg per tablet. It is manufactured by LEK S.A., Republic of Poland, and the Thai FDA lists Sandoz (Thailand) Ltd. as the licence holder.
Current Sandoz UK product information confirms that each film-coated tablet contains 10 mg Atorvastatin as atorvastatin calcium trihydrate. The reference tablet is described as a white to off-white, round, biconvex film-coated tablet marked “A10” on one side. (Medicines.org.uk)
Specified Thai pack size: 3 × 10 tablets = 30 tablets. (MIMS)
Atorvastatin is indicated as an adjunct to diet for reduction of elevated total cholesterol, LDL-C, ApoB, and triglycerides in adults, adolescents, and children aged 10 years or older with primary hypercholesterolaemia, including heterozygous familial hypercholesterolaemia, or combined/mixed hyperlipidaemia when diet and other non-pharmacological measures are inadequate. (Medicines.org.uk)
It is also indicated to reduce total cholesterol and LDL-C in adults with homozygous familial hypercholesterolaemia as an adjunct to other lipid-lowering treatments such as LDL apheresis, or when such treatments are unavailable. (Medicines.org.uk)
Atorvastatin is additionally indicated for prevention of cardiovascular events in adults considered at high risk of a first cardiovascular event, together with management of other modifiable risk factors. (Medicines.org.uk)
Patients should be placed on an appropriate cholesterol-lowering diet before treatment and should continue dietary measures throughout therapy.
The usual starting dose is:
Atorvastatin 10 mg once daily
The general dosage range is:
10–80 mg once daily
Dose selection should be individualised according to baseline LDL-C, treatment goals, and clinical response. Dose adjustments should generally be made at intervals of 4 weeks or longer, with a maximum recommended dose of 80 mg once daily. (Medicines.org.uk)
For primary hypercholesterolaemia and mixed hyperlipidaemia, the majority of patients are controlled with 10 mg once daily. A therapeutic response is generally evident within approximately 2 weeks, with the maximum effect of a given dose usually reached within approximately 4 weeks. (Medicines.org.uk)
Atorvastatin may be administered once daily at any time of day, with or without food.
Dose restrictions may be necessary in patients receiving interacting medicines or those at increased risk of statin-associated muscle toxicity.
Patients should promptly report unexplained muscle pain, tenderness, or weakness, particularly when accompanied by fever, marked fatigue, or dark urine, because myopathy or rhabdomyolysis may occur.
Patients with renal impairment, uncontrolled hypothyroidism, advanced age, or exposure to interacting medicines may have an increased risk of muscle toxicity.
Symptoms suggestive of hepatic injury, including jaundice, dark urine, loss of appetite, or unusual fatigue, require medical assessment.
Concomitant medicines should be reviewed carefully, particularly azole antifungals, macrolide antibiotics, antiviral agents, ciclosporin, fibrates, colchicine, and fusidic acid, because some combinations may substantially increase the risk of myopathy or rhabdomyolysis.
Patients who become pregnant during treatment should inform their healthcare professional promptly for reassessment of therapy.
The Thai FDA confirms that ATORVASTATIN SANDOZ GMBH 10 mg is a film-coated tablet, but the retrieved public Thai record does not clearly display product-specific storage instructions.
The Thai marketed product label and local package insert should therefore take precedence.
Although current Sandoz international product information provides storage guidance for its own marketed formulation, those instructions should not automatically be treated as Thai-product-specific unless confirmed on the Thai label.
