(Medicines.org.uk)

Mirtazapine 30 mg is an antidepressant indicated for the treatment of major depressive episodes in adults. It enhances central noradrenergic and serotonergic neurotransmission and has significant H1-histamine receptor antagonistic activity, which contributes to sedation and increased appetite. MERASAL registered in Thailand is a 30 mg film-coated tablet manufactured by Kenyaku (Thailand) Co., Ltd. (ข้อมูลยาแห่งชาติ)
Mirtazapine is a centrally active presynaptic α2-adrenergic antagonist that enhances central noradrenergic and serotonergic neurotransmission.
It blocks 5-HT2 and 5-HT3 receptors, thereby favouring serotonergic neurotransmission through 5-HT1 receptors, and also has clinically relevant H1-histamine receptor antagonistic activity. (Medicines.org.uk)
None.
Mirtazapine has no antibacterial, antiviral, or antifungal activity.
Mirtazapine is rapidly and well absorbed after oral administration, with an oral bioavailability of approximately 50%. Peak plasma concentrations are generally reached within approximately 2 hours, and food does not materially affect its pharmacokinetics.
Plasma protein binding is approximately 85%. Mirtazapine undergoes hepatic metabolism involving CYP1A2, CYP2D6, and CYP3A4.
The elimination half-life is generally approximately 20–40 hours, supporting once-daily dosing. (Medicines.org.uk)
Contraindicated in patients with hypersensitivity to Mirtazapine or any excipient.
Concomitant use with monoamine oxidase inhibitors (MAOIs), or use within 14 days of stopping an MAOI, is contraindicated. Approximately 14 days should also elapse after stopping Mirtazapine before initiating an MAOI. (DailyMed)
Patients should be closely monitored for clinical worsening, suicidal thoughts, or suicidal behaviour, particularly during early treatment and after dose changes.
Mirtazapine may cause substantial somnolence, dizziness, and impairment of alertness.
Rare cases of bone marrow suppression, granulocytopenia, and agranulocytosis have been reported. Fever, sore throat, stomatitis, or other signs of infection require prompt clinical assessment.
Careful use is required in patients with epilepsy, renal impairment, hepatic impairment, cardiovascular disease, hypotension, or diabetes.
Hyponatraemia, SIADH, akathisia, QT prolongation, and ventricular arrhythmias have also been reported.
Treatment should generally be tapered gradually rather than discontinued abruptly. (Medicines.org.uk)
Pregnancy: Human data remain limited. Treatment should be used only when the expected benefit justifies the potential risk, and neonates should be observed if exposure continues until late pregnancy.
Breastfeeding: Mirtazapine is excreted into human milk in small amounts. The decision to continue breastfeeding or treatment should be individualised.
Common adverse reactions include somnolence, sedation, dry mouth, increased appetite, weight gain, dizziness, and fatigue.
Other effects may include headache, abnormal dreams, tremor, orthostatic hypotension, constipation, and peripheral oedema.
Clinically important reactions include agranulocytosis, neutropenia, serotonin syndrome, hyponatraemia, seizures, mania/hypomania, QT prolongation, and severe cutaneous adverse reactions. (Medicines.org.uk)
Overdose may cause central nervous system depression, disorientation, prolonged sedation, tachycardia, and blood-pressure abnormalities.
Very high doses, particularly in mixed overdoses, may result in QT prolongation or serious cardiac arrhythmias.
There is no specific antidote. Management consists of symptomatic and supportive care, including vital-sign and ECG monitoring where clinically indicated. (DailyMed)
Mirtazapine must not be used concomitantly with MAO inhibitors.
Caution is required with serotonergic medicines including SSRIs, SNRIs, tramadol, buprenorphine, triptans, lithium, linezolid, methylene blue, and St. John’s Wort because of the risk of serotonin syndrome.
Alcohol, benzodiazepines, opioids, sedating antihistamines, and other CNS depressants may enhance sedation.
Carbamazepine and phenytoin can increase Mirtazapine clearance, while strong CYP3A4 inhibitors may increase exposure.
Caution is also warranted with medicines that prolong the QT interval, and INR monitoring should be considered when Mirtazapine is used with warfarin. (Medicines.org.uk)
Mirtazapine 30 mg per film-coated tablet
MERASAL 30 mg is registered in Thailand as a film-coated tablet containing Mirtazapine 30 mg per tablet. The Thai licence holder is Kenyaku (Thailand) Co., Ltd. (ข้อมูลยาแห่งชาติ)
The product is described as a brown, oval film-coated tablet marked “NRP” on one side and “MR 30” on the other. It is available in blisters of 10 tablets, with 3 or 10 blisters per carton. The specified 10×10 pack therefore contains 100 tablets. (Made in Thailand)
Indicated in adults for the treatment of major depressive episodes. (Medicines.org.uk)
The usual adult starting dose is:
15 mg or 30 mg daily
The usual effective dose is 15–45 mg/day, with a maximum recommended dose of 45 mg/day. (Medicines.org.uk)
Mirtazapine is generally administered once daily, preferably in the evening or immediately before bedtime.
MERASAL is a film-coated tablet and should be swallowed whole with sufficient fluid. It may be taken with or without food. (MIMS)
The antidepressant effect generally becomes apparent within 1–2 weeks, and an adequate dose should produce a positive response within approximately 2–4 weeks.
Once an optimal clinical response is achieved, treatment is generally continued for several months to maintain remission. Treatment should be gradually tapered when discontinued. (Medicines.org.uk)
Patients should be monitored for suicidal thoughts or behaviour, especially during early treatment and after dose changes. (DailyMed)
Fever, sore throat, oral ulcers, or other signs of infection require prompt assessment because they may indicate granulocytopenia or agranulocytosis.
Symptoms compatible with serotonin syndrome, including agitation, confusion, tremor, hyperreflexia, rigidity, sweating, or fever, require urgent medical assessment.
Treatment should be discontinued and medical assessment sought if jaundice develops.
Mirtazapine should generally be tapered gradually rather than stopped abruptly. (Medicines.org.uk)
The public Thai FDA record confirms MERASAL’s formulation and strength but does not display a product-specific storage condition. The marketed MERASAL label should therefore take precedence. (ข้อมูลยาแห่งชาติ)
Reference Mirtazapine 30 mg film-coated tablet labels vary: some specify storage in the original package, whereas others state that no special storage conditions are required. These conditions should not be transferred to MERASAL as product-specific instructions unless confirmed on its own label. (Medicines.org.uk)
