VALDOXAN (Agomelatine) 25 mg 28 Tablets
(Medicines.org.uk)
5-HT2C receptor antagonist
(Medicines.org.uk)
(Medicines.org.uk)
(Medicines.org.uk)
(Medicines.org.uk)
(ULN). (Medicines.org.uk)
(Medicines.org.uk)
(Medicines.org.uk)
(Medicines.org.uk)
(Medicines.org.uk)
(Medicines.org.uk)
(Medicines.org.uk)
(Medicines.org.uk)
(Medicines.org.uk)
(Medicines.org.uk)
(Medicines.org.uk)
(Medicines.org.uk)
(Medicines.org.uk)
(Medicines.org.uk)
(Medicines.org.uk)
(Medicines.org.uk)
(Medicines.org.uk)
Agomelatine 25 mg per film-coated tablet
(Medicines.org.uk)
(Medicines.org.uk)
(Medicines.org.uk)
(Medicines.org.uk)
(Medicines.org.uk)

VALDOXAN (Agomelatine) 25 mg 28 Tablets
Agomelatine 25 mg is an antidepressant indicated for the treatment of major depressive episodes in adults. It acts as an agonist at melatonin MT1 and MT2 receptors and as an antagonist at serotonin 5-HT2C receptors, influencing both mood regulation and circadian rhythms. (Medicines.org.uk)
Agomelatine is a melatonergic MT1/MT2 receptor agonist and 5-HT2C receptor antagonist.
It helps resynchronise circadian rhythms and increases noradrenaline and dopamine release in the frontal cortex without directly inhibiting monoamine reuptake. (Medicines.org.uk)
None.
Agomelatine has no antibacterial, antiviral, or antifungal activity.
Agomelatine is rapidly and well absorbed after oral administration, with absorption of at least approximately 80%. Absolute bioavailability is low, below 5% at therapeutic doses because of extensive first-pass metabolism.
Peak plasma concentrations occur within approximately 1–2 hours. Plasma protein binding is approximately 95%. (Medicines.org.uk)
It is metabolised predominantly through CYP1A2, with smaller contributions from CYP2C9 and CYP2C19. Its major metabolites are inactive and are eliminated mainly in urine.
Food does not materially affect bioavailability or the rate of absorption. (Medicines.org.uk)
Contraindicated in patients with hypersensitivity to Agomelatine or its excipients.
Contraindicated in hepatic impairment, including cirrhosis or active liver disease, and when baseline transaminases exceed 3 times the upper limit of normal. (Medicines.org.uk)
Concomitant administration with potent CYP1A2 inhibitors such as fluvoxamine or ciprofloxacin is contraindicated. (Medicines.org.uk)
The principal clinically important risk is drug-induced liver injury.
Liver function tests are required before treatment and at approximately 3, 6, 12, and 24 weeks, and thereafter when clinically indicated. The same monitoring schedule should be repeated following a dose increase. (Medicines.org.uk)
Treatment must be discontinued if serum transaminases exceed 3× ULN or if symptoms suggestive of hepatic injury develop.
Particular caution is required in patients with obesity, fatty liver disease, diabetes, substantial alcohol intake, or concomitant potentially hepatotoxic medicines.
Patients should also be monitored for suicidal thoughts or behaviour, particularly during early treatment and after dose changes. (Medicines.org.uk)
Pregnancy: Available human data are limited. As a precaution, use during pregnancy should preferably be avoided. (Medicines.org.uk)
Breastfeeding: It is not known whether Agomelatine or its metabolites are excreted into human milk. A decision should be made whether to discontinue breastfeeding or discontinue/avoid treatment based on the benefit-risk balance. (Medicines.org.uk)
The most commonly reported adverse effects are headache, nausea, and dizziness. They are generally mild to moderate and frequently occur during the first two weeks of therapy. (Medicines.org.uk)
Other adverse effects include somnolence, insomnia, anxiety, abnormal dreams, gastrointestinal symptoms, fatigue, and elevated hepatic transaminases.
Clinically important reactions include hepatitis, jaundice, hepatic failure, suicidal thoughts or behaviour, and mania/hypomania.
Reported symptoms include epigastric pain, somnolence, fatigue, agitation, anxiety, tension, dizziness, cyanosis, and malaise.
There is no specific antidote. Management consists of symptomatic treatment and routine clinical monitoring. (Medicines.org.uk)
Fluvoxamine and ciprofloxacin are potent CYP1A2 inhibitors and are contraindicated because they can markedly increase Agomelatine exposure. (Medicines.org.uk)
Caution is required with moderate CYP1A2 inhibitors such as propranolol and enoxacin.
Oestrogens may increase Agomelatine exposure.
Rifampicin may decrease Agomelatine bioavailability, while cigarette smoking, particularly 15 or more cigarettes per day, may reduce exposure by inducing CYP1A2. (Medicines.org.uk)
Concomitant use with alcohol is not advisable.
Agomelatine 25 mg per film-coated tablet
VALDOXAN 25 MG 28 Tablets is registered in Thailand as a film-coated tablet containing Agomelatine 25 mg per tablet. The Thai licence holder is Servier (Thailand) Ltd., and the manufacturer is Les Laboratoires Servier, France.
Pack size specified: 28 tablets
Indicated for the treatment of major depressive episodes in adults. (Medicines.org.uk)
Use is not recommended in patients under 18 years of age, and the current SmPC states that Agomelatine should not be used in patients aged 75 years or older because efficacy has not been demonstrated in this population. (Medicines.org.uk)
The recommended adult dose is:
25 mg once daily at bedtime
The tablet may be taken with or without food. (Medicines.org.uk)
If symptoms have not improved after approximately 2 weeks, the dose may be increased to:
50 mg once daily at bedtime, equivalent to two 25 mg tablets taken together.
Dose escalation requires an individual benefit-risk assessment because of the increased risk of transaminase elevation and requires renewed liver-function monitoring. (Medicines.org.uk)
Patients with depression should generally receive treatment for at least approximately 6 months to ensure that symptoms have resolved.
Dose tapering is not required when Agomelatine is discontinued according to the current SmPC. (Medicines.org.uk)
ALT and AST should be measured before treatment. Treatment must not be started if either exceeds 3× ULN.
Liver function should subsequently be assessed at approximately 3, 6, 12, and 24 weeks, and when clinically indicated. (Medicines.org.uk)
Treatment must be discontinued immediately if the patient develops symptoms suggestive of hepatic injury, including dark urine, pale stools, yellowing of the skin or eyes, right upper abdominal pain, or persistent unexplained new-onset fatigue, or if transaminases exceed 3× ULN. (Medicines.org.uk)
Concomitant use with fluvoxamine or ciprofloxacin is contraindicated.
Alcohol should be avoided during treatment. (Medicines.org.uk)
Patients should be monitored for suicidal ideation and behaviour, particularly during early treatment and after dose adjustments.
VALDOXAN 25 mg requires no special storage conditions.
Keep the medicine in its original packaging and out of the reach of children. (Medicines.org.uk)
