(Medicines.org.uk)
(Medicines.org.uk)
(Medicines.org.uk)

Atorvastatin 40 mg is an HMG-CoA reductase inhibitor (statin) used as an adjunct to diet to reduce LDL-C, total cholesterol, ApoB, and triglycerides and to reduce cardiovascular risk in appropriately selected patients. The Thai-registered CHLOVAS-ZIS 40 is a 40 mg Atorvastatin film-coated tablet, manufactured by Millimed BFS Co., Ltd., and supplied as 3 blisters of 10 tablets.
Atorvastatin is a selective competitive inhibitor of HMG-CoA reductase, the rate-limiting enzyme in hepatic cholesterol biosynthesis.
Reduction of intracellular hepatic cholesterol increases LDL receptor expression, enhancing uptake and clearance of circulating LDL-C. Atorvastatin also decreases hepatic VLDL production and consequently reduces LDL production. (Medicines.org.uk)
None.
Atorvastatin has no antibacterial, antiviral, or antifungal activity.
Atorvastatin is rapidly absorbed after oral administration, with peak plasma concentrations generally occurring within approximately 1–2 hours.
It undergoes extensive hepatic metabolism, predominantly through CYP3A4, producing pharmacologically active hydroxylated metabolites.
Atorvastatin is more than 98% protein bound. Its plasma elimination half-life is approximately 14 hours, although inhibitory activity against HMG-CoA reductase persists for approximately 20–30 hours because of active metabolites.
Elimination occurs predominantly through the bile following hepatic and extrahepatic metabolism. (Medicines.org.uk)
Contraindicated in patients with hypersensitivity to Atorvastatin or any excipient.
Contraindicated in patients with active liver disease or unexplained persistent serum transaminase elevations exceeding three times the upper limit of normal, according to the referenced SmPC. (Medicines.org.uk)
Pregnancy requirements differ between regulatory jurisdictions; locally approved Thai prescribing information should take precedence.
Atorvastatin may cause myalgia, myopathy, and rhabdomyolysis. Patients should promptly report unexplained muscle pain, tenderness, or weakness, particularly when accompanied by malaise or fever.
Risk increases with interacting medicines that raise Atorvastatin concentrations, including potent CYP3A4 inhibitors and certain hepatic transporter inhibitors.
Liver function should be assessed before treatment and thereafter when clinically indicated.
Rare immune-mediated necrotizing myopathy may persist despite statin discontinuation and can require immunosuppressive treatment.
Statins may cause modest increases in fasting glucose and HbA1c in susceptible patients. (Medicines.org.uk)
Pregnancy: Statin therapy is generally discontinued when pregnancy is recognised in most patients because treatment of hyperlipidaemia can usually be interrupted during pregnancy. Individual clinical assessment is appropriate in exceptional high-risk situations.
Breastfeeding: Atorvastatin is generally not recommended during breastfeeding because a clinically relevant risk to the nursing infant cannot be excluded. (DailyMed)
Adverse effects may include headache, myalgia, arthralgia, nausea, diarrhoea, constipation, dyspepsia, and other gastrointestinal symptoms.
Laboratory abnormalities may include elevations in hepatic transaminases or creatine kinase.
Clinically important adverse reactions include myopathy, rhabdomyolysis, hepatic injury, immune-mediated necrotizing myopathy, and hypersensitivity reactions.
Small increases in fasting glucose or HbA1c may occur in some patients. (Medicines.org.uk)
There is no specific antidote for Atorvastatin overdose.
Management consists of supportive and symptomatic care, with appropriate monitoring of hepatic, renal, muscular, and cardiovascular status.
Haemodialysis is unlikely to substantially enhance elimination because Atorvastatin is highly protein bound.
Atorvastatin is metabolised primarily through CYP3A4 and is also a substrate of hepatic transport proteins.
Strong CYP3A4 inhibitors such as clarithromycin, itraconazole, ketoconazole, posaconazole, and certain antiviral agents may substantially increase Atorvastatin exposure and the risk of myopathy or rhabdomyolysis. (Medicines.org.uk)
Concomitant use with glecaprevir/pibrentasvir should be avoided according to current product information.
Fibrates, ciclosporin, colchicine, and lipid-modifying doses of niacin may increase the risk of muscle toxicity.
Atorvastatin should not be administered concomitantly with systemic fusidic acid; temporary interruption of statin therapy may be required.
Certain medicines, including elbasvir/grazoprevir and letermovir, require Atorvastatin dose limitations. (Medicines.org.uk)
Large quantities of grapefruit juice may increase Atorvastatin concentrations and should be avoided.
Atorvastatin 40 mg per film-coated tablet
CHLOVAS-ZIS 40 is registered in Thailand as a film-coated tablet containing Atorvastatin 40 mg per tablet. The Thai FDA lists Millimed BFS Co., Ltd. as the licence holder/manufacturer.
The Thai MiT database describes it as a white, round, slightly convex film-coated tablet, marked “CZ” and “40” with a score line on one side and plain on the other. (Made in Thailand)
Specified pack size: 3 × 10 tablets = 30 tablets, supplied in aluminium-aluminium blister packs. (Made in Thailand)
Thai product information states that CHLOVAS-ZIS 40 is used with dietary control to reduce blood lipids and to help prevent coronary heart disease and stroke in high-risk patients. (Made in Thailand)
International Atorvastatin labeling includes treatment of primary hypercholesterolaemia, heterozygous familial hypercholesterolaemia, and combined or mixed hyperlipidaemia.
It is also used in adults with homozygous familial hypercholesterolaemia as an adjunct to other lipid-lowering therapies where appropriate.
Atorvastatin is additionally indicated for the prevention of cardiovascular events in adults at increased cardiovascular risk. (Medicines.org.uk)
Patients should be placed on an appropriate cholesterol-lowering diet before treatment and should continue dietary measures throughout therapy.
The usual adult starting dose is:
Atorvastatin 10 mg once daily
The general dosage range is:
10–80 mg once daily
Dosage should be individualised according to baseline LDL-C, treatment goals, cardiovascular risk, and clinical response. Dose adjustments should generally be made at intervals of 4 weeks or longer. (Medicines.org.uk)
For primary hypercholesterolaemia and mixed hyperlipidaemia, many patients are adequately controlled with 10 mg once daily.
For heterozygous familial hypercholesterolaemia, treatment generally begins at 10 mg/day and may be titrated to 40 mg/day and, where necessary, to 80 mg/day.
For homozygous familial hypercholesterolaemia, the referenced SmPC permits doses of approximately 10–80 mg/day as adjunctive therapy. (Medicines.org.uk)
Atorvastatin may be administered once daily at any time of day, with or without food.
CHLOVAS-ZIS 40 mg is a relatively high-strength tablet and is not the usual initial dose for most patients. It should be used according to medical assessment rather than started or increased to 40 mg without professional advice.
Patients should promptly report unexplained muscle pain, tenderness, or weakness, particularly when accompanied by fever, marked fatigue, or dark urine, because myopathy or rhabdomyolysis may occur. (DailyMed)
Patients with renal impairment, uncontrolled hypothyroidism, advanced age, or exposure to interacting medicines may have an increased risk of muscle toxicity.
Symptoms suggestive of hepatic injury, including jaundice, dark urine, loss of appetite, or unusual fatigue, require medical assessment.
Concomitant medicines should be reviewed carefully, particularly azole antifungals, macrolide antibiotics, antiviral agents, ciclosporin, fibrates, colchicine, and fusidic acid, because some combinations substantially increase the risk of myopathy or rhabdomyolysis. (Medicines.org.uk)
Patients who become pregnant during treatment should inform their healthcare professional promptly for reassessment of therapy.
The Thai FDA and MiT sources confirm that CHLOVAS-ZIS 40 is an Atorvastatin 40 mg film-coated tablet supplied in aluminium-aluminium blisters, but the retrieved public records do not clearly provide a product-specific storage condition.
The marketed CHLOVAS-ZIS 40 label and Thai product insert should therefore take precedence.
Reference Atorvastatin 40 mg products vary: some state that no special storage conditions are required, while others instruct storage in the original package to protect from moisture. These instructions should not be treated as CHLOVAS-ZIS-specific unless confirmed on its own label. (Medicines.org.uk)
