(Product Details)

PFIZER XARATOR 10MG 3×10 Tablets

HMG-CoA reductase

Atorvastatin overdose

PFIZER XARATOR 10MG 3×10 Tablets

Atorvastatin 10 mg is an HMG-CoA reductase inhibitor (statin) used as an adjunct to diet, exercise, and lifestyle modification to reduce LDL-C, total cholesterol, ApoB, and triglycerides and to help reduce cardiovascular risk in appropriately selected patients. The Thai FDA lists XARATOR (TABLETS 10 MG) as a film-coated tablet containing Atorvastatin 10 mg per tablet, licensed to Viatris Healthcare (Thailand) Ltd. The Thai product document confirms 10-tablet blister packs and 3-blister cartons.

Mechanism of Action

Atorvastatin is a selective competitive inhibitor of HMG-CoA reductase, the rate-limiting enzyme in hepatic cholesterol synthesis.

Reduced hepatic cholesterol increases LDL receptor expression, enhancing receptor-mediated clearance of circulating LDL-C. Atorvastatin also reduces hepatic VLDL production and consequently lowers LDL formation.

Antimicrobial Spectrum

None.

Atorvastatin has no antibacterial, antiviral, or antifungal activity.

Pharmacokinetics

Atorvastatin is rapidly absorbed after oral administration, with peak plasma concentrations generally reached within approximately 1–2 hours.

It undergoes extensive hepatic metabolism predominantly through CYP3A4, producing pharmacologically active metabolites.

Atorvastatin is more than 98% protein bound and has a plasma elimination half-life of approximately 14 hours, while HMG-CoA reductase inhibition persists longer because of active metabolites.

Elimination occurs predominantly through the bile after hepatic and extrahepatic metabolism.

Contraindications

Contraindicated in patients with hypersensitivity to Atorvastatin or any excipient.

The Thai XARATOR prescribing information contraindicates use in active liver disease or unexplained persistent serum transaminase elevations exceeding three times the upper limit of normal.

The same Thai product document contraindicates treatment during pregnancy, breastfeeding, and in women of childbearing potential not using adequate contraception.

Precautions

Atorvastatin may cause myalgia, myopathy, and rhabdomyolysis. Patients should promptly report unexplained muscle pain, tenderness, weakness, or cramps, particularly when accompanied by fever or malaise.

Risk is increased in patients with renal impairment, hypothyroidism, a history of statin- or fibrate-associated muscle toxicity, liver disease, or interacting medicines that increase Atorvastatin exposure.

The Thai XARATOR document reports dose-related transaminase elevations and recommends appropriate hepatic monitoring.

Rare immune-mediated necrotizing myopathy may persist despite statin discontinuation.

Statins may cause modest increases in blood glucose or HbA1c in susceptible patients.

Use During Pregnancy and Breastfeeding

Pregnancy: XARATOR is contraindicated during pregnancy according to the Thai prescribing information.

Breastfeeding: XARATOR is contraindicated during breastfeeding according to the same Thai product document.

Adverse Effects

Adverse effects may include headache, myalgia, arthralgia, nausea, diarrhoea, constipation, dyspepsia, and other gastrointestinal symptoms.

Laboratory abnormalities may include elevations in hepatic transaminases or creatine kinase.

Clinically important reactions include myopathy, rhabdomyolysis, hepatic injury, pancreatitis, hypersensitivity reactions, and immune-mediated necrotizing myopathy.

Overdose

There is no specific antidote for Atorvastatin overdose.

Management consists of supportive and symptomatic treatment, with appropriate monitoring of hepatic, renal, muscular, and cardiovascular status.

Haemodialysis is unlikely to substantially enhance elimination because Atorvastatin is highly protein bound.

Drug Interactions

Atorvastatin is metabolised predominantly through CYP3A4. CYP3A4 inhibitors may substantially increase Atorvastatin exposure and the risk of myopathy or rhabdomyolysis.

The Thai XARATOR prescribing information identifies interactions with clarithromycin, itraconazole, HIV protease inhibitors, HCV protease inhibitors, and letermovir, among others.

When co-administration with cyclosporine, telaprevir, tipranavir/ritonavir, or glecaprevir/pibrentasvir is necessary, the Thai product information states that Atorvastatin should not exceed 10 mg/day.

Excessive grapefruit juice consumption may increase Atorvastatin exposure and should be avoided.

Fibrates, colchicine, and certain other lipid-lowering medicines may further increase the risk of muscle toxicity.

Drug Name and Strength

Atorvastatin 10 mg per film-coated tablet

The Thai FDA identifies XARATOR (TABLETS 10 MG) as a film-coated tablet containing Atorvastatin 10 mg per tablet, licensed to Viatris Healthcare (Thailand) Ltd. and manufactured by Viatris Pharmaceuticals LLC.

The Thai local product document states that each tablet contains atorvastatin calcium in crystalline form equivalent to Atorvastatin 10 mg. The 10 mg tablet is white, round, and debossed “10” on one side and “ATV” on the other.

Specified pack size: 3 × 10 tablets = 30 tablets / box.

Indications

The Thai XARATOR prescribing information includes treatment, as an adjunct to diet, of primary hypercholesterolaemia, familial and non-familial hypercholesterolaemia, combined/mixed hyperlipidaemia, hypertriglyceridaemia, and dysbetalipoproteinaemia when response to dietary measures alone is inadequate.

It is also indicated to reduce total cholesterol and LDL-C in homozygous familial hypercholesterolaemia.

XARATOR is additionally indicated for prevention of cardiovascular complications, including reduction of the risk of fatal CHD, non-fatal myocardial infarction, stroke, revascularisation procedures, and angina in appropriately selected patients.

The Thai product information also includes selected indications in patients with CKD and in paediatric patients aged 10–17 years with heterozygous familial hypercholesterolaemia under defined criteria.

Recommended Dosage

Before treatment, hypercholesterolaemia should be addressed with appropriate diet, exercise, weight reduction where appropriate, and treatment of underlying medical conditions. These measures should continue during therapy.

The general adult dosage range is:

Atorvastatin 10–80 mg once daily

It may be administered at any time of day, with or without food.

For primary hypercholesterolaemia and mixed hyperlipidaemia, most patients are controlled with:

Atorvastatin 10 mg once daily

A therapeutic response is generally evident within approximately 2 weeks, with maximal response usually achieved within approximately 4 weeks.

Lipid levels should generally be reassessed within 2–4 weeks after initiation or dosage adjustment, with subsequent titration according to baseline LDL-C, treatment goals, and response.

For paediatric patients aged 10–17 years with heterozygous familial hypercholesterolaemia, the Thai product document recommends a starting dose of 10 mg/day and a maximum of 20 mg/day, with adjustments at intervals of at least 4 weeks.

No dosage adjustment is generally required in renal impairment according to the Thai prescribing information.

Warnings in Accordance with Public Safety Announcements

Patients should promptly report unexplained muscle pain, weakness, cramps, or tenderness, particularly when accompanied by fever, marked fatigue, or dark urine, because myopathy or rhabdomyolysis may occur.

Patients with renal impairment, hypothyroidism, a history of muscle disorders, or exposure to interacting medicines may have an increased risk of muscle toxicity.

Symptoms suggestive of hepatic injury, including jaundice, dark urine, loss of appetite, or unusual fatigue, require medical assessment.

Concomitant medicines should be reviewed carefully, particularly macrolide antibiotics, azole antifungals, HIV/HCV antivirals, ciclosporin, and other medicines affecting CYP3A4, because they may increase Atorvastatin exposure.

Pregnancy and breastfeeding are contraindications according to the Thai XARATOR prescribing information.

Storage Conditions

The Thai XARATOR prescribing information specifically states:

Store below 30°C.

The product is supplied in PA/Al/PVC-Al blisters containing 10 tablets, with cartons containing 3 or 10 blisters.
