Hydrochlorothiazide 50 mg is a thiazide diuretic used to treat hypertension and edema. It increases renal sodium and water excretion, thereby reducing extracellular fluid volume and helping lower blood pressure.
Hydrochlorothiazide is a thiazide diuretic that acts primarily in the distal renal tubule to reduce sodium and chloride reabsorption.
This increases urinary excretion of sodium, chloride, and water and may also increase potassium and bicarbonate loss.
Diuresis generally begins within approximately 2 hours, reaches its peak at about 4 hours, and lasts approximately 6–12 hours.
None.
Hydrochlorothiazide is not an antibiotic, antifungal, or antiviral agent and has no direct antimicrobial activity.
Hydrochlorothiazide is absorbed following oral administration and begins producing diuresis within approximately 2 hours.
It is not extensively metabolized and is eliminated primarily by the kidneys as unchanged drug. Reduced renal function may decrease drug clearance and increase the risk of adverse effects.
Hydrochlorothiazide is contraindicated in patients with anuria.
It is also contraindicated in patients with known hypersensitivity to Hydrochlorothiazide or the formulation. Caution is warranted in patients with a history of hypersensitivity to sulfonamide-derived medicines.
Serum sodium, potassium, magnesium, and calcium, renal function, and hydration status should be monitored as clinically appropriate.
Hydrochlorothiazide may cause hypokalemia, hyponatremia, hypomagnesemia, dehydration, and hypotension.
It may increase serum uric acid and precipitate gout and may impair glucose tolerance or worsen glycemic control.
Patients should also be advised to seek prompt medical assessment if acute eye pain, blurred vision, or a sudden change in vision develops.
Pregnancy: Hydrochlorothiazide crosses the placenta. Diuretics should not be routinely used to treat physiologic edema of pregnancy. Use during pregnancy should be restricted to situations in which there is a clear medical indication.
Breastfeeding: Hydrochlorothiazide is excreted into human milk. Higher doses may suppress lactation; therefore, the benefits and potential risks should be assessed individually.
Adverse effects may include increased urination, dizziness, orthostatic symptoms, hypotension, weakness, nausea, vomiting, loss of appetite, and electrolyte disturbances.
Clinically important abnormalities may include hypokalemia, hyponatremia, hyperuricemia, hyperglycemia, and dehydration.
Other reported effects include rash, photosensitivity, headache, muscle cramps, weakness, and erectile dysfunction. Rare serious adverse reactions include pancreatitis, blood dyscrasias, and severe hypersensitivity reactions.
Hydrochlorothiazide overdose may cause dehydration, hypotension, and electrolyte depletion, particularly involving potassium, sodium, and chloride.
There is no specific antidote. Treatment consists of supportive care, correction of fluid and electrolyte abnormalities, and monitoring of vital signs and renal function.
Concomitant antihypertensive therapy may produce an additive blood-pressure-lowering effect.
Hydrochlorothiazide may decrease renal clearance of lithium, increasing the risk of lithium toxicity.
NSAIDs may reduce its diuretic and antihypertensive effects and may increase the risk of renal impairment.
Hypokalemia may increase susceptibility to digoxin toxicity and cardiac arrhythmias.
Adjustment of insulin or other glucose-lowering therapy may be required because Hydrochlorothiazide may increase blood glucose levels.
Hydrochlorothiazide 50 mg
The Thai FDA database confirms multiple registered Hydrochlorothiazide 50 mg oral tablet products in Thailand.
Hydrochlorothiazide is indicated for the treatment of hypertension, either alone or in combination with other antihypertensive agents.
It is also indicated for the management of edema associated with conditions such as congestive heart failure, hepatic cirrhosis, corticosteroid or estrogen therapy, and certain renal disorders.
For hypertension, the usual initial adult dose is 25 mg once daily. The dose may be increased to 50 mg daily, administered as a single dose or in divided doses according to clinical response.
For edema, the usual adult dose is 25–100 mg daily, administered as a single or divided dose. Intermittent administration may be appropriate in some patients.
A 50 mg tablet strength does not mean that all patients should start treatment at 50 mg daily. Dosage should be individualized according to the indication, clinical response, renal function, blood pressure, and electrolyte status.
Patients should be monitored for dehydration, hypotension, and electrolyte disturbances, particularly hyponatremia and hypokalemia.
Hydrochlorothiazide may increase photosensitivity. Long-term cumulative exposure has also been associated in European safety reviews with an increased risk of certain types of non-melanoma skin cancer. Appropriate sun protection and monitoring of suspicious skin lesions are advisable.
Acute eye pain, blurred vision, or sudden visual deterioration requires prompt medical assessment.
Store at controlled room temperature in a tightly closed container, protected from light and moisture. U.S. labeling specifies 20–25°C (68–77°F). Follow the storage conditions stated on the specific product label.
