Fenofibrate overdose

LEXEMIN CAP 300MG 10×10 Capsules

Fenofibrate 300 mg is a fibric acid derivative (fibrate) used together with dietary, exercise, and weight-management measures, particularly for marked hypertriglyceridaemia. Its principal lipid-modifying effect is reduction of triglycerides and VLDL, with additional effects on HDL-C and other lipid parameters. The Thai FDA identifies LEXEMIN 300 as a hard capsule containing Fenofibrate 300 mg, manufactured by Unison Co., Ltd., while MIMS Thailand lists a pack size of 10 blisters × 10 capsules, total 100 capsules per box.

Mechanism of Action

Fenofibrate is a prodrug that is converted to fenofibric acid, its pharmacologically active metabolite.

Fenofibric acid activates peroxisome proliferator-activated receptor alpha (PPAR-α), increasing lipoprotein lipase activity and reducing ApoC-III production.

This enhances clearance of triglyceride-rich lipoproteins, reducing triglycerides and VLDL, while potentially reducing LDL-C and ApoB and modestly increasing HDL-C.

Antimicrobial Spectrum

None.

Fenofibrate has no antibacterial, antiviral, or antifungal activity.

Pharmacokinetics

Fenofibrate is rapidly hydrolysed after administration to fenofibric acid, the principal active metabolite.

Fenofibric acid is more than 99% albumin bound and has an elimination half-life of approximately 20 hours.

Elimination occurs predominantly through the urine as fenofibric acid and glucuronide conjugates.

Importantly, Fenofibrate pharmacokinetics are formulation-dependent. Conventional 300 mg capsules, micronised capsules, 160 mg tablets, and newer nanoparticle formulations should not automatically be considered dose-equivalent.

Contraindications

Fenofibrate is contraindicated in patients with hypersensitivity to the active substance or formulation excipients.

Current reference labeling contraindicates treatment in severe hepatic impairment, persistent unexplained liver-function abnormalities, and severe renal impairment.

It is also contraindicated in patients with known gallbladder disease and generally in patients with chronic or acute pancreatitis, except acute pancreatitis secondary to severe hypertriglyceridaemia.

The specific excipient list for the Thai LEXEMIN 300 capsule should be checked when evaluating product-specific allergy risks.

Precautions

Fenofibrate may cause elevations in hepatic transaminases, and liver-function monitoring is appropriate.

A reversible increase in serum creatinine may occur, making renal monitoring particularly important in older adults, patients with diabetes, and those with impaired renal function.

Myopathy and rhabdomyolysis may occur, with greater risk in renal impairment, hypothyroidism, advanced age, or concomitant statin therapy.

Fenofibrate can increase biliary cholesterol excretion and consequently the risk of gallstone formation.

Pancreatitis has also been reported during fibrate treatment.

Use During Pregnancy and Breastfeeding

Pregnancy: Human data are limited. Fenofibrate should generally be used during pregnancy only when the expected benefit is judged to outweigh potential risk.

Breastfeeding: Available information is insufficient to exclude risk to a nursing infant; Fenofibrate is generally not recommended during breastfeeding.

Adverse Effects

Commonly reported effects include abdominal pain, nausea, vomiting, diarrhoea, flatulence, and other gastrointestinal symptoms.

Laboratory abnormalities may include increased hepatic transaminases, serum creatinine, and homocysteine.

Clinically important reactions include myopathy, rhabdomyolysis, gallstones, pancreatitis, hepatitis, and hypersensitivity reactions.

Venous thromboembolic events have also been reported, although they are uncommon.

Overdose

There is no specific antidote for Fenofibrate overdose.

Management consists of supportive and symptomatic treatment, with monitoring of renal, hepatic, and muscular status as clinically indicated.

Haemodialysis is unlikely to substantially enhance elimination because fenofibric acid is highly protein bound.

Drug Interactions

Statins: Concomitant use may increase the risk of myopathy and rhabdomyolysis, requiring careful benefit-risk assessment and monitoring.

Warfarin and other oral anticoagulants: Fenofibrate may potentiate anticoagulant effects; INR monitoring and dose adjustment may be necessary.

Ciclosporin: Severe reversible renal impairment has been reported during concomitant use.

Thiazolidinediones: Marked paradoxical reductions in HDL-C have occasionally been reported when used with fibrates.

Fenofibrate may also inhibit CYP2C9 to some extent, requiring caution with narrow-therapeutic-index medicines metabolised through this pathway.

Drug Name and Strength

Fenofibrate 300 mg per hard capsule

The Thai FDA identifies LEXEMIN 300 as a hard capsule containing Fenofibrate 300 mg per capsule, manufactured by Unison Co., Ltd.

MIMS Thailand describes the capsule as white and orange, marked “Lex” and “300”, and confirms a 10 × 10 capsule pack. (MIMS)

A regulatory record for the same Unison-manufactured product lists Fenofibrate 300 mg capsules in ALU-ALU packaging, 10 capsules per blister and 10 blisters per box. (FDD)

Indications

A regulatory record for LEXEMIN 300 identifies its use as an adjunct to diet in adults with very high serum triglyceride concentrations, particularly Type IV and V hyperlipidaemia, who have responded inadequately to dietary measures and are at risk of pancreatitis. (FDD)

More broadly, Fenofibrate is used for severe hypertriglyceridaemia and selected cases of mixed dyslipidaemia when non-pharmacological measures are inadequate.

Thai-market indications should follow the locally approved LEXEMIN 300 prescribing information where they differ from international Fenofibrate labeling.

Recommended Dosage

Fenofibrate dosage is highly formulation-dependent.

A conventional 300 mg capsule should not automatically be treated as equivalent to a 160 mg tablet, 200 mg micronised capsule, or 145 mg nanoparticle formulation.

The Thai FDA record confirms LEXEMIN 300 as a 300 mg capsule but does not publicly display the complete local dosing instructions.

For this reason, the LEXEMIN 300 package insert should be used for the exact dose and food instructions, rather than applying dosing from other Fenofibrate formulations.

Renal function should be evaluated before treatment because dose reduction or avoidance may be necessary in renal impairment.

Warnings in Accordance with Public Safety Announcements

Patients should seek assessment for unexplained muscle pain, weakness, tenderness, or dark urine, especially when receiving a statin concomitantly.

Renal function should be monitored in older adults, patients with diabetes, and those with reduced kidney function.

Symptoms suggesting hepatic or biliary disease, including jaundice, dark urine, loss of appetite, or right-upper-quadrant pain, require medical assessment.

Severe upper abdominal pain with nausea, vomiting, or radiation to the back warrants assessment for pancreatitis or gallstone disease.

Storage Conditions

The Thai FDA confirms the product identity, strength, dosage form, and manufacturer of LEXEMIN 300, but the retrieved Thai public record does not clearly provide a product-specific storage temperature.

A regulatory record for the same Unison-manufactured product lists a 3-year shelf life but does not specify the storage temperature. The Thai marketed package label and local package insert should therefore take precedence. (FDD)
