Bethanechol Chloride 5 mg

Bethanechol Chloride 5 mg

(DailyMed)

Bethanechol Chloride

(Recommended Dosage)

(DailyMed)

(Warnings in Accordance with Public Safety Announcements)

(DailyMed)

English Version

Bethanechol Chloride 5 mg

What is Bethanechol Chloride 5 mg?

Bethanechol Chloride 5 mg is a parasympathomimetic medicine that increases urinary bladder contraction. It is used for nonobstructive urinary retention, including postoperative, postpartum, and neurogenic bladder-related urinary retention. (DailyMed)

Mechanism of Action

Bethanechol Chloride is a direct-acting cholinergic agonist that produces parasympathetic effects primarily through stimulation of muscarinic receptors.

It increases the tone and contractility of the urinary bladder detrusor muscle, producing contractions that may initiate micturition and facilitate bladder emptying. It also increases gastrointestinal tone and motility.

Bethanechol does not significantly cross the blood-brain barrier because it is a charged quaternary ammonium compound. (DailyMed)

Antimicrobial Spectrum

None.

Bethanechol Chloride is not an antibiotic, antifungal, or antiviral agent and has no direct antimicrobial activity.

Pharmacokinetics

Following oral administration, effects may begin within approximately 30 minutes, are generally maximal within 60–90 minutes, and may persist for approximately 1 hour.

Bethanechol does not cross the blood-brain barrier to a clinically significant extent because of its charged quaternary amine structure.

The official prescribing information states that the metabolic rate and mode of excretion have not been fully elucidated. (DailyMed)

Contraindications

Bethanechol Chloride is contraindicated in patients with hypersensitivity to the drug and in those with hyperthyroidism, peptic ulcer disease, latent or active bronchial asthma, pronounced bradycardia or hypotension, vasomotor instability, coronary artery disease, epilepsy, or parkinsonism.

It should not be used in the presence of mechanical gastrointestinal or urinary obstruction, bladder neck obstruction, acute inflammatory gastrointestinal disease, peritonitis, marked vagotonia, or when the integrity of the gastrointestinal or bladder wall is uncertain. (DailyMed)

Precautions

In patients with urinary retention, if the urinary sphincter fails to relax while the bladder contracts, urine may reflux into the ureters and renal pelvis. In the presence of bacteriuria, this may contribute to reflux infection.

Dizziness, lightheadedness, or fainting may occur, particularly when rising from a sitting or lying position.

Bethanechol should preferably be taken 1 hour before or 2 hours after meals to reduce the likelihood of nausea and vomiting. (DailyMed)

Use During Pregnancy and Breastfeeding

Pregnancy: Adequate reproductive studies are lacking. Bethanechol should be used during pregnancy only when clearly needed and when the expected benefit justifies the potential risk.

Breastfeeding: It is not known whether Bethanechol is excreted in human milk. A decision should be made whether to discontinue breastfeeding or discontinue the medicine, taking into account the importance of treatment to the mother. (DailyMed)

Adverse Effects

Reported adverse effects include abdominal cramps or discomfort, colicky pain, nausea, belching, diarrhea, increased bowel sounds, excessive salivation, urinary urgency, headache, flushing, sweating, and a sensation of warmth.

Cardiovascular effects may include decreased blood pressure with reflex tachycardia. Bronchial constriction and asthmatic attacks may occur.

Lacrimation and miosis have also been reported. Adverse effects become more likely as the dose increases. (DailyMed)

Overdose

Early manifestations of overdose may include abdominal discomfort, excessive salivation, flushing, sweating, nausea, and vomiting.

Atropine sulfate is the specific antidote for excessive Bethanechol effects. Administration of atropine and management of overdose should be performed under medical supervision. (DailyMed)

Drug Interactions

Particular caution is required when Bethanechol is administered with ganglion-blocking agents, as a critical fall in blood pressure may occur.

Other cholinergic medicines may enhance its pharmacological effects, whereas anticholinergic agents such as atropine oppose its effects. (DailyMed)

Drug Name and Strength

Bethanechol Chloride 5 mg

Bethanechol Chloride 5 mg oral tablets are registered in Thailand according to the Thai FDA drug database.

Indications

Bethanechol Chloride is indicated for acute postoperative and postpartum nonobstructive (functional) urinary retention.

It is also indicated for neurogenic atony of the urinary bladder with urinary retention. (DailyMed)

Recommended Dosage

Dosage should be individualized according to the type and severity of the condition.

The usual adult oral dose is 10–50 mg three or four times daily.

To determine the minimum effective dose, treatment may be initiated with 5–10 mg and titrated according to clinical response under medical supervision.

Bethanechol should preferably be administered 1 hour before or 2 hours after meals.

Therefore, 5 mg represents a tablet strength and may be used during initial dose titration; it does not mean that the usual therapeutic regimen is 5 mg once daily. (DailyMed)

Warnings in Accordance with Public Safety Announcements

No specific recent public safety announcement for Bethanechol Chloride 5 mg was identified in the official sources reviewed beyond the established prescribing-information warnings.

Bethanechol must not be used to stimulate bladder emptying in the presence of mechanical urinary obstruction, because increasing bladder contraction against an obstruction may cause harm.

Prompt medical assessment is required if severe bronchospasm, marked hypotension, syncope, or significant cholinergic toxicity develops. (DailyMed)

Storage Conditions

Store at 20–25°C (68–77°F); temperature excursions between 15–30°C (59–86°F) are permitted. Store in a tightly closed, light-resistant container. (DailyMed)
