Drospirenone และ Ethinylestradiol กด hypothalamic–pituitary–ovarian axis ทำให้การหลั่ง FSH และ LH ลดลง จึง ยับยั้งการตกไข่

เนื่องจาก Drospirenone มี antimineralocorticoid activity หากได้รับปริมาณสูงมากควรพิจารณาติดตาม serum potassium, sodium และ acid-base status
ไม่มี specific antidote การรักษาเป็นแบบ supportive และ symptomatic
ยาหรือสมุนไพรที่เหนี่ยวนำ hepatic enzymes โดยเฉพาะ CYP3A4 อาจลดระดับฮอร์โมนและลดประสิทธิภาพการคุมกำเนิด เช่น rifampicin, rifabutin, carbamazepine, phenytoin, phenobarbital, primidone, oxcarbazepine และ St. John’s Wort จึงอาจจำเป็นต้องใช้วิธีคุมกำเนิดเสริม. (DailyMed)
Combined hormonal contraceptives สามารถลดระดับ lamotrigine และอาจทำให้ควบคุมอาการชักได้แย่ลง
ควรระวังการใช้ร่วมกับ spironolactone, eplerenone, potassium-sparing diuretics, potassium supplements, ACE inhibitors และ ARBs เนื่องจากอาจเพิ่มความเสี่ยง hyperkalaemia

Drospirenone 3 mg + Ethinylestradiol 20 micrograms is a combined oral contraceptive (COC) used primarily to prevent pregnancy. It suppresses ovulation, increases cervical mucus viscosity, and produces endometrial changes that reduce the likelihood of conception. The combination suppresses the hypothalamic-pituitary-ovarian axis and thereby inhibits ovulation.
Drospirenone additionally possesses antiandrogenic and antimineralocorticoid activity, the latter being clinically relevant to serum potassium monitoring in susceptible patients.

YAZ has no antibacterial, antiviral, or antifungal activity and does not protect against HIV or other sexually transmitted infections.
Drospirenone is rapidly absorbed after oral administration, is highly albumin bound, and undergoes extensive metabolism.
Ethinylestradiol is rapidly absorbed but undergoes significant presystemic and first-pass metabolism.

YAZ is contraindicated in patients with renal impairment, adrenal insufficiency, a high risk of arterial or venous thrombotic disease, undiagnosed abnormal uterine bleeding, breast cancer, liver tumours, or significant liver disease under current U.S. prescribing information. (DailyMed)
Estrogen-containing contraception should also be avoided in patients with conditions such as migraine with aura or other major thrombotic risk states.
Combined hormonal contraceptives increase the risk of venous and arterial thromboembolism.
Drospirenone-containing COCs may be associated with a higher VTE risk than some levonorgestrel-containing products.
Drospirenone’s potassium-sparing effect may increase serum potassium in susceptible patients, particularly those with renal or adrenal disease or taking potassium-increasing medicines.
Pregnancy: Discontinue YAZ if pregnancy occurs.
Breastfeeding: Combined hormonal contraceptives can reduce milk production and are generally not preferred early postpartum. (DailyMed)
Frequently reported adverse effects include headache or migraine, menstrual irregularities, nausea or vomiting, breast pain or tenderness, and mood changes. (DailyMed)
Serious adverse reactions include thromboembolic events, stroke, myocardial infarction, hepatic disorders, and hyperkalaemia in susceptible patients.
Overdose may cause nausea, vomiting, and vaginal bleeding.
There is no specific antidote. Supportive care is indicated, with electrolyte monitoring after substantial Drospirenone exposure where clinically appropriate.
Enzyme-inducing medicines and herbal products may decrease hormonal exposure and reduce contraceptive efficacy. Examples include rifampicin, carbamazepine, phenytoin, barbiturates, oxcarbazepine, and St. John’s Wort. (DailyMed)

Potassium-increasing medicines including spironolactone, eplerenone, ACE inhibitors, ARBs, potassium-sparing diuretics, and potassium supplements require additional caution.
