English Version

Hydrochlorothiazide 50 mg

What is Hydrochlorothiazide 50 mg?

Hydrochlorothiazide 50 mg is a thiazide diuretic used to treat hypertension and edema. It increases renal excretion of sodium, chloride, and water, reducing extracellular fluid volume and helping lower blood pressure. The 50 mg strength does not mean that all patients should begin treatment at 50 mg daily. (DailyMed)

Mechanism of Action

Hydrochlorothiazide is a thiazide diuretic that acts primarily on the distal renal tubular mechanism of electrolyte reabsorption.

It increases the excretion of sodium and chloride and may also increase the urinary loss of potassium and bicarbonate.

Following oral administration, diuresis generally begins within approximately 2 hours, peaks at about 4 hours, and lasts approximately 6–12 hours. (DailyMed)

Antimicrobial Spectrum

None.

Hydrochlorothiazide is not an antibiotic, antifungal, or antiviral medicine and has no direct antimicrobial activity.

Pharmacokinetics

Hydrochlorothiazide is absorbed following oral administration and is not significantly metabolized. It is eliminated primarily by the kidneys as unchanged drug.

Reduced renal function may decrease its clearance and increase the risk of fluid and electrolyte disturbances. (DailyMed)

Contraindications

Hydrochlorothiazide is contraindicated in patients with anuria and in patients with known hypersensitivity to Hydrochlorothiazide or any component of the formulation.

A history of hypersensitivity to sulfonamide-derived medicines should also be considered.

Precautions

Serum sodium, potassium, magnesium, calcium, renal function, blood pressure, and hydration status should be monitored as clinically appropriate.

Hydrochlorothiazide may cause hyponatremia, hypokalemia, hypomagnesemia, dehydration, and hypotension.

It may increase serum uric acid and precipitate gout and may impair glucose tolerance or worsen glycemic control.

Use During Pregnancy and Breastfeeding

Pregnancy: Hydrochlorothiazide crosses the placenta. Diuretics should not be routinely used for physiologic edema during pregnancy. Treatment should be reserved for situations with a clear medical indication.

Breastfeeding: Hydrochlorothiazide is excreted into human milk. Higher doses of thiazide diuretics may reduce milk production; therefore, the benefits and potential risks should be assessed individually.

Adverse Effects

Adverse effects may include increased urination, dizziness, orthostatic symptoms, hypotension, weakness, nausea, vomiting, and muscle cramps.

Clinically important abnormalities include hypokalemia, hyponatremia, hyperuricemia, hyperglycemia, and dehydration.

Other reported reactions include rash, photosensitivity, headache, muscle weakness, and, rarely, pancreatitis, blood dyscrasias, or serious hypersensitivity reactions. (DailyMed)

Overdose

Overdose may cause dehydration, hypotension, and electrolyte depletion, particularly involving sodium, potassium, and chloride.

There is no specific antidote. Treatment consists of supportive care and correction of fluid and electrolyte disturbances.

Drug Interactions

Other antihypertensive medicines may enhance the blood-pressure-lowering effect of Hydrochlorothiazide.

Hydrochlorothiazide may decrease renal clearance of lithium, increasing the risk of lithium toxicity.

NSAIDs may reduce its diuretic and antihypertensive effects and may increase the risk of renal impairment in susceptible patients.

Hypokalemia may increase susceptibility to digoxin toxicity and cardiac arrhythmias.

Drug Name and Strength

Hydrochlorothiazide 50 mg

The Thai FDA database confirms registered Hydrochlorothiazide 50 mg oral tablets in Thailand.

Indications

Hydrochlorothiazide is indicated for the treatment of hypertension, either alone or in combination with other antihypertensive medicines.

It is also indicated for the management of edema associated with conditions including congestive heart failure, hepatic cirrhosis, corticosteroid or estrogen therapy, and certain renal disorders. (DailyMed)

Recommended Dosage

For hypertension, the usual initial adult dose is 25 mg once daily. The dose may be increased to 50 mg daily, administered as a single dose or in divided doses according to clinical response.

For edema, the usual adult dose is 25–100 mg daily, administered as a single or divided dose.

A 50 mg tablet strength does not mean that all patients should start treatment at 50 mg daily. Dosage should be individualized according to the indication, clinical response, renal function, blood pressure, and electrolyte status. (DailyMed)

Warnings in Accordance with Public Safety Announcements

Patients should be monitored for dehydration and electrolyte disturbances, particularly hyponatremia and hypokalemia.

Marked weakness, severe dizziness, syncope, muscle weakness or cramps, palpitations, confusion, or substantially reduced urine output requires medical assessment.

Appropriate sun protection is advisable because photosensitivity may occur.

Storage Conditions

Store at controlled room temperature, generally 20–25°C (68–77°F), in a tightly closed container protected from light and moisture. Some U.S. labeling permits excursions between 15–30°C (59–86°F). Follow the storage conditions stated on the specific product label. (DailyMed)
