(Product Details)

ATOZET 10MG/10MG 3×10 Tablets

(Medicines.org.uk)

(Medicines.org.uk)

ATOZET 10MG/10MG 3×10 Tablets

ATOZET 10 mg/10 mg is a fixed-dose lipid-lowering combination containing Ezetimibe 10 mg plus Atorvastatin 10 mg per tablet. Ezetimibe reduces intestinal cholesterol absorption, while Atorvastatin inhibits hepatic cholesterol synthesis. The Thai-registered ATOZET (10MG/10MG) is a film-coated tablet licensed to Organon (Thailand) Ltd.

Mechanism of Action

Ezetimibe selectively inhibits the intestinal cholesterol transporter Niemann-Pick C1-Like 1 (NPC1L1) at the brush border of the small intestine, thereby reducing intestinal absorption of cholesterol and plant sterols.

Atorvastatin competitively inhibits HMG-CoA reductase, the rate-limiting enzyme in hepatic cholesterol synthesis. Reduced hepatic cholesterol increases LDL receptor expression and enhances clearance of circulating LDL-C.

These complementary mechanisms reduce cholesterol through both intestinal absorption and hepatic synthesis.

Antimicrobial Spectrum

None.

Ezetimibe and Atorvastatin have no antibacterial, antiviral, or antifungal activity.

Pharmacokinetics

Ezetimibe is absorbed after oral administration and rapidly converted to the active metabolite ezetimibe-glucuronide, with enterohepatic recycling contributing to its prolonged pharmacological effect.

Atorvastatin is rapidly absorbed and undergoes extensive hepatic metabolism, primarily through CYP3A4, to pharmacologically active metabolites.

No clinically significant pharmacokinetic interaction has been demonstrated between Ezetimibe and Atorvastatin when administered together. (Medicines.org.uk)

Contraindications

Contraindicated in patients with hypersensitivity to Ezetimibe, Atorvastatin, or any excipient.

Contraindicated in patients with active liver disease or unexplained persistent serum transaminase elevations exceeding three times the upper limit of normal.

According to the referenced SmPC, Ezetimibe/Atorvastatin is contraindicated during pregnancy and breastfeeding, and in women of childbearing potential who are not using appropriate contraception.

Concomitant administration with glecaprevir/pibrentasvir is contraindicated. (Medicines.org.uk)

Precautions

Atorvastatin may rarely cause myalgia, myositis, myopathy, and rhabdomyolysis. Patients should promptly report unexplained muscle pain, tenderness, or weakness, particularly when accompanied by fever or malaise. (Medicines.org.uk)

Liver function should be assessed before treatment and thereafter when clinically indicated. Persistent transaminase elevations exceeding three times the upper limit of normal may require dose reduction or discontinuation. (Medicines.org.uk)

Use caution in patients who consume substantial quantities of alcohol or have a history of liver disease.

The fixed-dose combination is not recommended in moderate-to-severe hepatic impairment. (Medicines.org.uk)

Statins may modestly increase blood glucose in susceptible patients; appropriate metabolic monitoring should be considered.

Use During Pregnancy and Breastfeeding

Pregnancy: Ezetimibe/Atorvastatin is contraindicated during pregnancy according to the referenced SmPC. Treatment should be suspended if pregnancy occurs or is suspected. (Medicines.org.uk)

Breastfeeding: The combination is contraindicated during breastfeeding according to the referenced product information because a clinically significant risk to the nursing infant cannot be excluded. (Medicines.org.uk)

Adverse Effects

Adverse effects may include headache, myalgia, diarrhoea, nausea, abdominal discomfort, flatulence, fatigue, and elevations in hepatic transaminases.

Clinically important reactions include myopathy, rhabdomyolysis, hepatitis, pancreatitis, hypersensitivity reactions, hyperglycaemia, and rare interstitial lung disease. (Medicines.org.uk)

Overdose

There is no specific antidote for Ezetimibe/Atorvastatin overdose.

Management consists of supportive and symptomatic treatment, with appropriate assessment of hepatic, muscular, renal, and cardiovascular function.

Haemodialysis is unlikely to substantially increase Atorvastatin elimination because of its high degree of plasma protein binding.

Drug Interactions

Atorvastatin is metabolised primarily by CYP3A4 and is also a substrate of several hepatic transport proteins. Medicines that increase Atorvastatin exposure may increase the risk of myopathy and rhabdomyolysis. (Medicines.org.uk)

Strong CYP3A4 inhibitors such as clarithromycin, ketoconazole, itraconazole, voriconazole, and posaconazole, as well as certain antiviral agents, may markedly increase Atorvastatin concentrations.

Glecaprevir/pibrentasvir is contraindicated. (Medicines.org.uk)

Concomitant use with fibrates is not recommended in the referenced SmPC because of the increased risk of muscle toxicity. (Medicines.org.uk)

Caution is required with ciclosporin, and ciclosporin concentrations should be monitored where appropriate.

When Ezetimibe/Atorvastatin is added to warfarin, another coumarin anticoagulant, or fluindione, INR should be appropriately monitored. (Medicines.org.uk)

When used with a bile acid sequestrant, ATOZET should be administered at least 2 hours before or at least 4 hours after the sequestrant. (Medicines.org.uk)

Atorvastatin-containing treatment should not be co-administered with systemic fusidic acid; statin treatment should be suspended during systemic fusidic acid therapy according to current SmPC guidance. (Medicines.org.uk)

Drug Name and Strength

Ezetimibe 10 mg + Atorvastatin 10 mg per film-coated tablet

ATOZET (10MG/10MG) is registered in Thailand as a film-coated tablet containing Ezetimibe and Atorvastatin at a strength of 10 mg + 10 mg per tablet. The Thai FDA lists Organon (Thailand) Ltd. as the licence holder.

Specified pack size: 3 × 10 tablets = 30 tablets

Indications

Ezetimibe/Atorvastatin is used as an adjunct to diet in adults with primary hypercholesterolaemia or mixed hyperlipidaemia when combination therapy with Ezetimibe and Atorvastatin is appropriate.

The current fixed-dose-combination SmPC used as an international reference specifies use in adults who are already adequately controlled with Ezetimibe and Atorvastatin administered separately at the same dose level, including substitution therapy to reduce cardiovascular-event risk in patients with coronary heart disease and a history of acute coronary syndrome. (Medicines.org.uk)

The locally approved Thai ATOZET prescribing information should take precedence for local use.

Recommended Dosage

The available fixed-dose combination range is generally:

Ezetimibe/Atorvastatin 10/10 mg to 10/80 mg once daily

Patients should remain on an appropriate lipid-lowering diet throughout treatment. (Medicines.org.uk)

ATOZET 10 mg/10 mg is taken once daily and may be administered at any time of day, with or without food.

The current fixed-dose-combination SmPC used as the international reference states that the combination is not suitable for initial therapy. The individual components should first be adjusted to the appropriate doses, after which the patient may be switched to the corresponding fixed-dose strength. (Medicines.org.uk)

No dosage adjustment is generally required in elderly patients or patients with renal impairment.

Use cautiously in hepatic impairment and do not use in active liver disease. (Medicines.org.uk)

Warnings in Accordance with Public Safety Announcements

Patients should promptly report unexplained muscle pain, tenderness, or weakness, particularly when associated with fever or malaise, because myopathy or rhabdomyolysis may occur. (Medicines.org.uk)

Symptoms suggestive of liver injury, including jaundice, dark urine, marked fatigue, loss of appetite, or right-upper-quadrant discomfort, require medical assessment.

Ezetimibe/Atorvastatin is contraindicated in active liver disease or persistent unexplained serum transaminase elevations exceeding three times the upper limit of normal. (Medicines.org.uk)

Concomitant treatment with glecaprevir/pibrentasvir is contraindicated.

Systemic fusidic acid should not be administered concomitantly with Atorvastatin-containing therapy; statin treatment should generally be suspended during systemic fusidic acid treatment. (Medicines.org.uk)

Pregnancy and breastfeeding are contraindications according to the referenced SmPC. (Medicines.org.uk)

Storage Conditions

The Thai FDA confirms the formulation and strength of ATOZET 10 mg/10 mg, but product-specific storage instructions are not displayed on the retrieved public Thai record.

A current Ezetimibe/Atorvastatin 10 mg/10 mg SmPC states that no special temperature storage conditions are required and the tablets should be stored in the original package to protect from light. (Medicines.org.uk)

Storage instructions supplied with the Thai-market ATOZET product should take precedence if they differ.
