(DailyMed)
(DailyMed)
(DailyMed)
(DailyMed)
(DailyMed)
(MIMS)
(DailyMed)
(DailyMed)
(DailyMed)
(DailyMed)
(DailyMed)
(DailyMed)
(DailyMed)
Pitavastatin overdose
(DailyMed)
(DailyMed)
(DailyMed)
1 mg/day
2 mg/day. (DailyMed)
(DailyMed)

Pitavastatin 2 mg is an HMG-CoA reductase inhibitor (statin) used with dietary and lifestyle measures to reduce LDL-C and other atherogenic lipids in appropriately selected patients. LIVALO 2 mg supplied in Thailand is manufactured by Kowa Company Ltd., Japan; MIMS Thailand lists Biopharm as the distributor and a pack size of 10 blisters × 10 tablets, total 100 tablets per box.
Pitavastatin is a competitive inhibitor of HMG-CoA reductase, the rate-limiting enzyme in hepatic cholesterol synthesis.
Reduced intracellular hepatic cholesterol increases LDL receptor expression, enhancing uptake and clearance of circulating LDL-C. This results in reductions in LDL-C, total cholesterol, ApoB, and non-HDL-C, with additional effects on triglycerides and HDL-C.
Unlike several other statins, Pitavastatin undergoes only minimal cytochrome P450 metabolism. (DailyMed)
None.
Pitavastatin has no antibacterial, antiviral, or antifungal activity.
Pitavastatin reaches peak plasma concentrations approximately 1 hour after oral administration. The absolute bioavailability of the oral solution is approximately 51%. (DailyMed)
Its principal metabolic pathway is glucuronidation through UGT1A3 and UGT2B7, followed by formation of pitavastatin lactone.
Cytochrome P450 metabolism is minimal, involving primarily CYP2C9 and, to a lesser extent, CYP2C8.
Approximately 79% of an administered radiolabelled dose is recovered in faeces and approximately 15% in urine. The mean plasma elimination half-life is approximately 12 hours. (DailyMed)
Current LIVALO prescribing information contraindicates treatment in patients with hypersensitivity to Pitavastatin or any excipient.
Concomitant cyclosporine is contraindicated because it substantially increases Pitavastatin exposure and the risk of myopathy and rhabdomyolysis.
LIVALO is also contraindicated in patients with acute liver failure or decompensated cirrhosis. (DailyMed)
Thai public warning information additionally states that Pitavastatin should not be used during pregnancy or breastfeeding or in patients with liver disease; the locally approved Thai product information should therefore take precedence. (MIMS)
Pitavastatin may cause myalgia, myopathy, and rhabdomyolysis, including rare cases complicated by acute kidney injury secondary to myoglobinuria.
Risk factors include age 65 years or older, uncontrolled hypothyroidism, renal impairment, interacting medicines, and higher Pitavastatin doses. (DailyMed)
Patients should promptly report unexplained muscle pain, tenderness, or weakness, especially when associated with malaise or fever.
Rare immune-mediated necrotizing myopathy (IMNM) may persist despite discontinuation and may require specialist treatment. (DailyMed)
Liver enzymes should be assessed before treatment and thereafter when clinically indicated.
Statin therapy, including LIVALO, may cause increases in fasting serum glucose and HbA1c in susceptible patients.
Pregnancy: Current U.S. LIVALO labeling advises patients of potential fetal risk and recommends reassessment of therapy if pregnancy occurs. Thai public warning information states that Pitavastatin should not be used during pregnancy; for the Thai product, local prescribing information should take precedence. (DailyMed)
Breastfeeding: Current LIVALO labeling states that breastfeeding is not recommended during treatment. Thai public warning information likewise advises against use during breastfeeding. (DailyMed)
The most frequently reported adverse reactions in current LIVALO clinical-trial labeling include myalgia, constipation, diarrhoea, back pain, and pain in the extremities. (DailyMed)
Elevations in creatine kinase or hepatic enzymes may occur.
Clinically important adverse reactions include myopathy, rhabdomyolysis, hepatic dysfunction, hypersensitivity reactions, and immune-mediated necrotizing myopathy.
There is no specific antidote for Pitavastatin overdose.
Management consists of supportive and symptomatic treatment, with appropriate monitoring of hepatic, renal, and muscular status.
Haemodialysis is unlikely to be beneficial because Pitavastatin is highly protein bound. (DailyMed)
Cyclosporine: Concomitant use is contraindicated. Cyclosporine markedly increases Pitavastatin exposure. (DailyMed)
Gemfibrozil: Concomitant treatment is not recommended because of increased myopathy and rhabdomyolysis risk.
Fibrates, colchicine, and lipid-modifying doses of niacin may further increase the risk of muscle toxicity. (DailyMed)
Erythromycin: Current U.S. labeling limits LIVALO to 1 mg once daily when used concomitantly.
Rifampin: LIVALO should not exceed 2 mg once daily when co-administered. (DailyMed)
Because Pitavastatin is minimally dependent on CYP3A4 metabolism, its interaction profile differs from that of Simvastatin and Atorvastatin, although all concomitant medicines should still be reviewed before treatment.
Pitavastatin 2 mg per film-coated tablet
The Thai FDA identifies LIVALO (TABLET 2 MG) as a Pitavastatin 2 mg product manufactured by Kowa Company Ltd., Nagoya Factory, Japan.
The current original LIVALO label specifies pitavastatin calcium 2.09 mg equivalent to Pitavastatin 2 mg per tablet. (DailyMed)
MIMS Thailand identifies Kowa as manufacturer, Biopharm as distributor, and the Thai pack size as 10 × 10 tablets = 100 tablets per box. (MIMS)
LIVALO is indicated as an adjunct to diet to reduce LDL-C in adults with primary hyperlipidaemia.
Current U.S. labeling also includes adults and paediatric patients aged 8 years and older with heterozygous familial hypercholesterolaemia (HeFH). (DailyMed)
Clinical studies demonstrate reductions in LDL-C, ApoB, total cholesterol, and non-HDL-C, with additional effects on triglycerides and HDL-C in patients with primary hyperlipidaemia or mixed dyslipidaemia. (DailyMed)
Indications specifically approved for the Thai-market product should follow the locally approved Thai prescribing information where this differs from foreign labeling.
According to current LIVALO prescribing information, the general dosage range is:
Pitavastatin 2–4 mg once daily
Maximum recommended dosage:
4 mg once daily
LIVALO may be administered once daily with or without food, preferably at approximately the same time each day. (DailyMed)
LIVALO 2 mg is therefore a standard therapeutic strength for appropriately selected patients.
LDL-C may be reassessed as early as approximately 4 weeks after initiation, with dosage adjustment according to treatment goals and response.
For patients with moderate or severe renal impairment or end-stage renal disease receiving haemodialysis, current U.S. labeling recommends a starting dose of 1 mg once daily and a maximum dose of 2 mg once daily. (DailyMed)
If high-intensity statin therapy is required or LDL-C goals are not achieved with LIVALO 4 mg/day, the current label recommends considering an alternative LDL-C-lowering treatment rather than exceeding the 4 mg maximum. (DailyMed)
Thai public warning information for Pitavastatin states that the medicine should not be used during pregnancy or breastfeeding or in patients with liver disease, and advises patients who develop muscle pain in the calves, back, or throughout the body to stop treatment and seek medical advice. (MIMS)
Patients should promptly seek assessment for unexplained muscle pain, weakness, tenderness, dark urine, or symptoms accompanied by fever or marked fatigue.
Symptoms suggesting hepatic injury, including jaundice, dark urine, loss of appetite, or right upper abdominal discomfort, require medical assessment.
Concomitant treatment with cyclosporine, erythromycin, rifampin, gemfibrozil, fibrates, colchicine, or lipid-modifying doses of niacin should be reviewed carefully because avoidance or dosage adjustment may be required. (DailyMed)
The retrieved Thai public records confirm the product identity, strength, manufacturer, and dosage form, but do not clearly display product-specific storage instructions for the Biopharm-distributed Thai pack.
The Thai marketed package label and local package insert should therefore take precedence.
For reference, the current U.S. LIVALO label specifies storage at 15–30°C and protection from light; these instructions should not automatically replace Thai-market storage requirements if the local label differs. (DailyMed)
