What is Ethinylestradiol 20 micrograms + Desogestrel 150 micrograms?

Ethinylestradiol 20 micrograms + Desogestrel 150 micrograms is a combined oral contraceptive used to prevent pregnancy. It primarily suppresses ovulation and additionally alters cervical mucus and the endometrium. MERCILON 28 contains 21 active tablets followed by 7 placebo tablets. (MIMS)

Mechanism of Action

This medicine is a combined oral contraceptive (COC) containing ethinylestradiol as the estrogen component and desogestrel as the progestogen component.

Desogestrel is converted to its active metabolite, etonogestrel. Contraceptive efficacy is achieved primarily through suppression of gonadotropin secretion and inhibition of ovulation, with additional effects on cervical mucus and the endometrium. (Medicines.org.uk)

This medicine has no antibacterial, antiviral, or antifungal activity and does not protect against HIV or other sexually transmitted infections.

Pharmacokinetics

Desogestrel is rapidly absorbed and converted to etonogestrel. Peak serum concentrations occur at approximately 1.5 hours, and bioavailability is approximately 62–81%.

Contraindications

Combined hormonal contraceptives are contraindicated in women with current or previous venous thromboembolism, significant thrombophilia, or a high risk of VTE.

Use During Pregnancy and Breastfeeding

Pregnancy: This medicine is not indicated during pregnancy and should be discontinued if pregnancy occurs.

Adverse Effects

Reported adverse effects include nausea, headache, migraine, breast tenderness, abdominal discomfort, mood changes, spotting, and breakthrough bleeding.

Drug Interactions

Recommended Dosage

Take one tablet once daily at approximately the same time each day, following the order shown on the blister pack.

MERCILON 28 uses a 21+7 regimen: 21 active tablets followed by 7 placebo tablets. The next pack should be started immediately after the final placebo tablet, without an additional tablet-free interval, even if withdrawal bleeding has not completely stopped. (MIMS)

When initiated on the first day of menstrual bleeding, contraceptive protection begins immediately. If started on days 2–5, an additional barrier method should be used during the first 7 days. (MIMS)

Missing a placebo tablet does not directly reduce contraceptive efficacy, but the missed placebo tablet should be discarded to avoid delaying the start of the next pack.
