ฤทธิ์คุมกำเนิดเกิดจากการทำงานร่วมกันของ Ethinylestradiol และ Drospirenone โดยกด hypothalamic–pituitary–ovarian axis ทำให้การหลั่ง FSH และ LH ลดลง ส่งผลให้ ยับยั้งการตกไข่

ประเด็นสำคัญของยาคุมฮอร์โมนรวมคือความเสี่ยง venous thromboembolism และ arterial thromboembolism ควรประเมินปัจจัยเสี่ยงก่อนเริ่มยา เช่น อายุ การสูบบุหรี่ obesity ประวัติ thrombosis ในครอบครัว การผ่าตัดหรือการไม่เคลื่อนไหวนาน ความดันโลหิตสูง และ migraine

การให้นมบุตร: COC อาจลดปริมาณน้ำนมและเปลี่ยนองค์ประกอบของน้ำนม จึงมักไม่ใช่ตัวเลือกแรกในระยะแรกหลังคลอด โดยต้องพิจารณาร่วมกับความเสี่ยง VTE หลังคลอดและความต้องการให้นมบุตร

ไม่มี specific antidote การรักษาเป็นแบบ supportive และ symptomatic
ยาที่เหนี่ยวนำ hepatic enzymes อาจลดระดับฮอร์โมนและลดประสิทธิภาพการคุมกำเนิด เช่น rifampicin, rifabutin, carbamazepine, phenytoin, phenobarbital, primidone, oxcarbazepine และ St. John’s Wort
COC สามารถลดระดับ lamotrigine และอาจทำให้ควบคุมอาการชักได้แย่ลง
เนื่องจาก Drospirenone มี potassium-sparing activity ควรระวังเมื่อใช้ร่วมกับ spironolactone, eplerenone, potassium-sparing diuretics, potassium supplements, ACE inhibitors, ARBs และยาบางชนิดที่เพิ่ม potassium โดยเฉพาะผู้ที่มี renal impairment. (Medicines.org.uk)

ก่อนเริ่มใช้ควรประเมินปัจจัยเสี่ยงต่อ thrombosis, migraine, ความดันโลหิต โรคตับ การสูบบุหรี่ โรคไต และยาที่ใช้ร่วมกัน
สำหรับแผง 21 เม็ด ให้รับประทาน วันละ 1 เม็ด เวลาใกล้เคียงกันทุกวัน ติดต่อกัน 21 วัน

ยาคุมฮอร์โมนรวมเพิ่มความเสี่ยง DVT และ pulmonary embolism โดยเฉพาะในผู้ที่มีปัจจัยเสี่ยงเดิม
การสูบบุหรี่เพิ่มความเสี่ยง cardiovascular อย่างมีนัยสำคัญ โดยเฉพาะในผู้หญิงอายุมากกว่า 35 ปี จึงควรพิจารณาวิธีคุมกำเนิดที่ไม่มี estrogen ในกลุ่มนี้. (DailyMed)
หากมี ขาบวมและเจ็บข้างเดียว เจ็บหน้าอกเฉียบพลัน หายใจลำบาก ไอเป็นเลือด แขนขาอ่อนแรง พูดไม่ชัด หรือปวดศีรษะรุนแรงผิดปกติ ควรหยุดยาและเข้ารับการประเมินทันที

Drospirenone 3 mg + Ethinylestradiol 30 micrograms is a combined oral contraceptive (COC) used to prevent pregnancy. The contraceptive effect results primarily from suppression of the hypothalamic-pituitary-ovarian axis and inhibition of ovulation.

This medicine has no antibacterial, antiviral, or antifungal activity and does not protect against HIV or other sexually transmitted infections.
Drospirenone is rapidly absorbed following oral administration and reaches steady-state concentrations after approximately 8 days of repeated dosing. (Medicines.org.uk)

The principal serious safety concern is venous and arterial thromboembolism.
Drospirenone has aldosterone-antagonist activity and may increase potassium in susceptible patients. Serum potassium monitoring should be considered during the first treatment cycle in patients at risk, particularly those with renal impairment or receiving potassium-sparing medicines. Pregnancy: Treatment should be discontinued if pregnancy is confirmed.
Breastfeeding: Combined oral contraceptives may reduce breast-milk production and are generally not first-line contraception early postpartum.
Adverse effects may include headache, nausea, breast tenderness, mood changes, abdominal discomfort, and breakthrough bleeding.
Rare but clinically important adverse reactions include DVT, pulmonary embolism, myocardial infarction, stroke, and hepatic disorders.
Hyperkalaemia may occur in susceptible patients.
Overdose may cause nausea and withdrawal bleeding.
Because Drospirenone has antimineralocorticoid activity, potassium, sodium, and acid-base status should be monitored after substantial overdose. (DailyMed)
There is no specific antidote.
Enzyme-inducing medicines may reduce contraceptive efficacy, including rifampicin, rifabutin, carbamazepine, phenytoin, phenobarbital, primidone, oxcarbazepine, and St. John’s Wort.
Combined hormonal contraceptives may substantially reduce lamotrigine concentrations.

Take 1 tablet once daily at approximately the same time for 21 consecutive days.

If a tablet is taken less than 12 hours late, contraceptive protection is generally maintained. The missed tablet should be taken as soon as remembered.
