Ethinylestradiol 15 micrograms + Gestodene 60 micrograms

What is Ethinylestradiol 15 micrograms + Gestodene 60 micrograms?

Ethinylestradiol 15 micrograms + Gestodene 60 micrograms is a low-dose combined oral contraceptive used to prevent pregnancy. It primarily suppresses ovulation and also alters cervical mucus and the endometrium.

Mechanism of Action

This medicine is a combined oral contraceptive (COC) containing ethinylestradiol as the estrogen component and gestodene as the progestogen component.

Its contraceptive effect is achieved mainly through suppression of the hypothalamic-pituitary-ovarian axis and inhibition of ovulation. It also increases the viscosity of cervical mucus and produces endometrial changes that contribute to contraceptive efficacy.

Antimicrobial Spectrum

None.

This medicine has no antibacterial, antiviral, or antifungal activity and does not protect against HIV or other sexually transmitted infections.

Pharmacokinetics

Gestodene is rapidly absorbed following oral administration and has high systemic bioavailability. It binds to albumin and sex hormone-binding globulin and undergoes hepatic metabolism.

Ethinylestradiol is rapidly absorbed but undergoes extensive first-pass metabolism in the intestinal wall and liver, with partial enterohepatic circulation.

Contraindications

Combined hormonal contraceptives are contraindicated in women with current or previous venous thromboembolism, significant thrombophilia, or a high risk of venous thrombosis.

They are also contraindicated in women with arterial thromboembolic disease, myocardial infarction, stroke or TIA, migraine with focal neurological symptoms, severe hypertension, diabetes with vascular complications, severe liver disease, liver tumours, hormone-dependent malignancy, or unexplained genital bleeding.

Precautions

The principal safety concern is venous and arterial thromboembolism.

Gestodene-containing combined contraceptives may be associated with a higher risk of VTE than certain levonorgestrel-containing contraceptives. Individual thrombotic risk factors should therefore be assessed before treatment.

Smoking, increasing age, obesity, migraine, hypertension, major surgery, prolonged immobilisation, and a personal or family history of thrombosis are particularly important factors.

Use During Pregnancy and Breastfeeding

Pregnancy: This medicine is not indicated during pregnancy and should be discontinued if pregnancy occurs.

Breastfeeding: Combined hormonal contraceptives may reduce breast-milk production and alter milk composition, particularly during the early postpartum period.

Adverse Effects

Reported adverse effects include nausea, headache, breast tenderness, abdominal discomfort, mood changes, spotting, and breakthrough bleeding.

Serious but uncommon adverse reactions include deep-vein thrombosis, pulmonary embolism, myocardial infarction, and stroke.

Overdose

Overdose may cause nausea, vomiting, and vaginal bleeding.

There is no specific antidote. Treatment is supportive and symptomatic.

Drug Interactions

Hepatic enzyme-inducing medicines may lower contraceptive hormone concentrations and reduce effectiveness. Important examples include rifampicin, rifabutin, carbamazepine, phenytoin, phenobarbital, primidone, oxcarbazepine, topiramate, and St. John’s Wort.

Certain HIV and hepatitis C treatments may also alter hormone exposure.

Combined hormonal contraceptives may reduce plasma concentrations of lamotrigine, potentially affecting seizure control.

Drug Name and Strength

Ethinylestradiol 15 micrograms + Gestodene 60 micrograms per active tablet

The specified Exeltis-MINIDOZ 28 S’ product is registered in Thailand as a film-coated tablet containing Ethinylestradiol 15 mcg + Gestodene 60 mcg, supplied in a 28-tablet pack. (ข้อมูลยาแห่งชาติ)

Each pack contains 24 yellow active tablets, each containing Gestodene 60 mcg + Ethinylestradiol 15 mcg, followed by 4 white inactive tablets. (PRD MIMS Web)

Indications

Indicated for oral contraception in women.

Thai product information additionally lists reduction of inflammatory and non-inflammatory acne lesions in women who also require contraception. (PRD MIMS Web)

Recommended Dosage

Take one tablet once daily at approximately the same time each day, following the order shown on the blister pack.

MINIDOZ uses a 24+4 regimen: 24 consecutive days of active hormone tablets followed by 4 inactive tablets. The next pack is started immediately after the final inactive tablet, without any additional tablet-free interval. (PRD MIMS Web)

Starting on the first day of menstrual bleeding provides immediate contraceptive protection. If initiation occurs later, additional barrier contraception may be required depending on the circumstances.

Management of missed active tablets depends on the number of tablets missed and where in the cycle the omission occurs. Avoid unnecessarily prolonging the hormone-free interval.

Missing an inactive tablet does not directly reduce contraceptive efficacy, but the missed inactive tablet should be discarded so that the next pack is not started late.

Warnings in Accordance with Public Safety Announcements

The most important safety concern is venous and arterial thromboembolism.

Unilateral leg swelling or pain, sudden unexplained shortness of breath, chest pain, haemoptysis, sudden severe headache, unilateral weakness or numbness, speech disturbance, or sudden visual changes require urgent medical assessment.

This contraceptive does not protect against HIV or other sexually transmitted infections.

Storage Conditions

Store in the original packaging, protected from excessive heat, moisture, and direct sunlight. Follow the storage instructions on the specific MINIDOZ product labeling.
