Norelgestromin 6 mg + Ethinylestradiol 0.6 mg Transdermal Patch

What is Norelgestromin + Ethinylestradiol Transdermal Patch?

Norelgestromin 6 mg + Ethinylestradiol 0.6 mg transdermal patch is a combined hormonal contraceptive used once weekly to prevent pregnancy. One patch is worn each week for three consecutive weeks, followed by one patch-free week. (Medicines.org.uk)

Mechanism of Action

The combination suppresses gonadotropin secretion through the estrogenic and progestogenic actions of ethinylestradiol and norelgestromin, thereby preventing ovulation.

It also alters cervical mucus and produces changes in the endometrium that contribute to contraceptive effectiveness. (European Medicines Agency (EMA))

Antimicrobial Spectrum

None.

This contraceptive patch does not provide protection against HIV or other sexually transmitted infections and has no antimicrobial activity.

Pharmacokinetics

Each 20 cm² patch contains 6 mg norelgestromin and 600 micrograms ethinylestradiol and releases an average of approximately 203 micrograms of norelgestromin and 33.9 micrograms of ethinylestradiol per 24 hours.

Serum concentrations reach a plateau at approximately 48 hours after application. Following patch removal, the mean elimination half-lives are approximately 28 hours for norelgestromin and 17 hours for ethinylestradiol. (Medicines.org.uk)

Contraindications

The patch is contraindicated in women with a current or previous venous thromboembolism, significant thrombophilia, arterial thromboembolic disease, migraine with focal neurological symptoms, severe hypertension, diabetes with vascular complications, breast cancer or other estrogen-dependent malignancy, significant hepatic disease, hepatic tumours, or unexplained genital bleeding. (Medicines.org.uk)

Precautions

The principal safety concern is the increased risk of venous and arterial thromboembolism associated with combined hormonal contraception.

Risk assessment is particularly important in women who smoke, especially those older than 35 years, and in those with obesity, hypertension, migraine, prolonged immobilisation, or a significant personal or family history of thrombosis. (Medicines.org.uk)

Contraceptive efficacy may be reduced in women weighing 90 kg or more. (Medicines.org.uk)

Use During Pregnancy and Breastfeeding

Pregnancy: The patch is not indicated during pregnancy. If pregnancy occurs, treatment should be discontinued immediately. (Medicines.org.uk)

Breastfeeding: Combined hormonal contraceptives may reduce the quantity and alter the composition of breast milk. Current SmPC information therefore does not recommend use until the mother has completely weaned her child. (Medicines.org.uk)

Adverse Effects

The most commonly reported adverse reactions are headache, nausea, and breast tenderness. Spotting and breakthrough bleeding may occur, particularly during the first few cycles. (Medicines.org.uk)

Serious but uncommon adverse reactions include venous thrombosis, pulmonary embolism, myocardial infarction, and stroke. (Medicines.org.uk)

Overdose

Overexposure may cause nausea, vomiting, or vaginal bleeding.

All transdermal contraceptive patches should be removed if overdose is suspected, and treatment is supportive and symptomatic. There is no specific antidote. (Medicines.org.uk)

Drug Interactions

Hepatic enzyme-inducing medicines such as carbamazepine, phenytoin, barbiturates, primidone, rifampicin, modafinil, oxcarbazepine, topiramate, and St. John’s Wort may reduce contraceptive effectiveness. (Medicines.org.uk)

For short-term use of enzyme-inducing medicines, an additional barrier method should generally be used during treatment and for 28 days after discontinuation. (Medicines.org.uk)

Combined hormonal contraceptives may also reduce plasma concentrations of lamotrigine, potentially reducing seizure control. (Medicines.org.uk)

Drug Name and Strength

Norelgestromin 6 mg + Ethinylestradiol 0.6 mg per transdermal patch

Each patch releases an average of 203 micrograms of norelgestromin and 33.9 micrograms of ethinylestradiol per 24 hours. (Medicines.org.uk)

Indications

Indicated for female contraception in women of reproductive age. Clinical safety and efficacy have been established primarily in women aged 18–45 years. (Medicines.org.uk)

Recommended Dosage

Apply one patch once weekly for 3 consecutive weeks, replacing it on the same day each week.

Week 1: first patch
Week 2: second patch
Week 3: third patch
Week 4: patch-free interval

The patch-free interval must never exceed 7 days. (Medicines.org.uk)

If a patch has been detached for less than 24 hours, reapply it or replace it immediately; no additional contraception is required.

If it has been detached for 24 hours or longer, or the duration is unknown, start a new contraceptive cycle with a new patch and use a non-hormonal backup method for 7 days. (Medicines.org.uk)

Warnings in Accordance with Public Safety Announcements

Combined hormonal contraceptives increase the risk of venous and arterial thromboembolic events.

Unilateral leg swelling or pain, sudden unexplained shortness of breath, chest pain, haemoptysis, sudden neurological weakness, severe unusual headache, speech disturbance, or sudden visual changes require urgent medical assessment. (Medicines.org.uk)

Storage Conditions

Store in the original package to protect from light and moisture.

Do not refrigerate or freeze. Apply immediately after removal from the protective sachet. (Medicines.org.uk)
