อาการข้างเคียง

BIOPHARM LIPZ 28 Tablets

Drospirenone 3 mg + Ethinylestradiol 30 micrograms is a combined oral contraceptive (COC) used to prevent pregnancy through inhibition of ovulation, increased cervical mucus viscosity, and endometrial changes.

Mechanism of Action

The contraceptive effect results primarily from suppression of the hypothalamic-pituitary-ovarian axis and prevention of ovulation.

Drospirenone also possesses antimineralocorticoid and antiandrogenic properties, which are clinically relevant to its potassium-related precautions.

Antimicrobial Spectrum

None.

This medicine has no antibacterial, antiviral, or antifungal activity and does not protect against HIV or other sexually transmitted infections.

Pharmacokinetics

Both Drospirenone and Ethinylestradiol are absorbed after oral administration and undergo hepatic metabolism before elimination as metabolites in urine and faeces.

Drospirenone has antimineralocorticoid activity, which contributes to the potential for increased serum potassium in susceptible patients.

Contraindications

Combined hormonal contraception is contraindicated in patients with current or previous venous thromboembolism, arterial thromboembolism, major thrombophilia, migraine with aura, severe cardiovascular disease, severe hepatic disease, hepatic tumours, hormone-dependent malignancy, or unexplained vaginal bleeding.

Precautions

Combined hormonal contraceptives increase the risk of venous and arterial thromboembolism.

For Drospirenone-containing COCs, the estimated VTE rate is approximately 9–12 cases per 10,000 users per year, compared with approximately 5–7 cases per 10,000 users per year for levonorgestrel-, norethisterone-, or norgestimate-containing COCs. (Medicines.org.uk)

Risk is further increased by smoking, increasing age, obesity, a family history of thrombosis, major surgery, prolonged immobilisation, and the postpartum period.

Use During Pregnancy and Breastfeeding

Pregnancy: Contraindicated. Treatment should be discontinued if pregnancy is confirmed.

Adverse Effects

Reported adverse effects include headache, dizziness, nausea, vomiting, breast tenderness, breakthrough bleeding, menstrual changes, migraine, and depressed mood.

Overdose

Overdose may cause nausea, vomiting, and withdrawal bleeding.

Drug Interactions

Indications

Indicated for oral contraception to prevent pregnancy.

Recommended Dosage

For LIPZ ED 28 tablets, take 1 tablet once daily in the order shown on the blister, at approximately the same time each day, until all 28 tablets have been taken. Start the next pack the following day without a tablet-free interval.

Tablets 1–21 are active tablets, followed by 7 placebo tablets.

Patients with previous DVT/PE, thrombophilia, migraine with aura, stroke, myocardial infarction, or multiple major cardiovascular risk factors should not use estrogen-containing contraception without appropriate assessment.
