(Medicines.org.uk)

(MIMS)

(MIMS)

(MIMS)

(MIMS)

St. John’s Wort. (MIMS)

English Version

Ethinylestradiol 20 micrograms + Desogestrel 150 micrograms

What is Ethinylestradiol 20 micrograms + Desogestrel 150 micrograms?

Ethinylestradiol 20 micrograms + Desogestrel 150 micrograms is a low-dose combined oral contraceptive used to prevent pregnancy. It primarily inhibits ovulation and also alters cervical mucus and the endometrium. MINNY 28 uses a 21 active + 7 inactive tablet regimen. (MIMS)

Mechanism of Action

This medicine is a combined oral contraceptive (COC) containing ethinylestradiol as the estrogen component and desogestrel as the progestogen component.

Desogestrel is converted to its active metabolite, etonogestrel. Contraceptive efficacy is achieved primarily by suppression of FSH and LH and inhibition of ovulation, with additional effects on cervical mucus and the endometrium. (MIMS)

Antimicrobial Spectrum

None.

This medicine has no antibacterial, antiviral, or antifungal activity and does not protect against HIV or other sexually transmitted infections.

Pharmacokinetics

Desogestrel is rapidly absorbed and converted to etonogestrel. Peak concentrations occur at approximately 1.5 hours, bioavailability is approximately 62–81%, and the terminal half-life of etonogestrel is approximately 30 hours. (Medicines.org.uk)

Ethinylestradiol is rapidly absorbed but undergoes substantial first-pass metabolism in the intestinal wall and liver and undergoes partial enterohepatic recirculation.

Contraindications

Combined hormonal contraceptives are contraindicated in women with current or previous venous thromboembolism, significant thrombophilia, or a high risk of VTE.

They are also contraindicated in women with arterial thromboembolic disease, myocardial infarction, stroke or TIA, migraine with focal neurological symptoms, uncontrolled hypertension, diabetes with vascular complications, severe liver disease, liver tumours, hormone-dependent malignancy, unexplained genital bleeding, or pregnancy. (MIMS)

Precautions

The principal safety concerns are venous thromboembolism and arterial thromboembolism.

Desogestrel-containing combined hormonal contraceptives are associated with a higher VTE risk than some levonorgestrel-containing combined contraceptives. Individual thrombotic risk factors should therefore be assessed before treatment. (Medicines.org.uk)

Smoking substantially increases cardiovascular risk, particularly in women older than 35 years. (MIMS)

Use During Pregnancy and Breastfeeding

Pregnancy: This medicine is not indicated during pregnancy and should be discontinued if pregnancy occurs.

Breastfeeding: Combined oral contraceptives may reduce the quantity and alter the composition of breast milk, particularly during the postpartum period. (MIMS)

Adverse Effects

Reported adverse effects include nausea, vomiting, headache, dizziness, migraine, breast tenderness, mood changes, weight changes, spotting, and breakthrough bleeding.

Serious but uncommon adverse reactions include deep-vein thrombosis, pulmonary embolism, myocardial infarction, stroke, hypertension, and hepatic disorders. (MIMS)

Overdose

Overdose may cause nausea, vomiting, and vaginal or withdrawal bleeding.

There is no specific antidote. Management is supportive and symptomatic. (MIMS)

Drug Interactions

Hepatic enzyme-inducing medicines may reduce contraceptive hormone concentrations and decrease effectiveness. Important examples include rifamycins, carbamazepine, phenytoin, barbiturates, and St. John’s Wort. (MIMS)

Certain HIV and hepatitis C medicines may also affect hormone exposure, and combined hormonal contraceptives may reduce plasma concentrations of lamotrigine.
