(Product Details)

BIOPHARM LIPZ 21 Tablets

(Medicines.org.uk)

(Medicines.org.uk)

(Medicines.org.uk)

ห้ามใช้ในผู้ที่มี thrombophilia ที่สำคัญ, migraine with aura, severe hypertension, diabetes ที่มี vascular complications, severe liver disease, hepatic tumour, known or suspected hormone-dependent malignancy หรือ unexplained vaginal bleeding ที่ยังไม่ได้รับการวินิจฉัย

(Medicines.org.uk)

St. John’s Wort

English Version

Drospirenone 3 mg + Ethinylestradiol 30 micrograms

What is Drospirenone 3 mg + Ethinylestradiol 30 mcg?

Drospirenone 3 mg + Ethinylestradiol 30 micrograms is a combined oral contraceptive (COC) used to prevent pregnancy through suppression of ovulation, increased cervical mucus viscosity, and endometrial changes.

For BIOPHARM LIPZ 21 Tablets, all 21 tablets are active hormone-containing tablets; there are no placebo tablets in this pack. (MIMS)

Mechanism of Action

The contraceptive effect is primarily mediated by suppression of the hypothalamic-pituitary-ovarian axis and inhibition of ovulation.

Drospirenone also possesses antiandrogenic and mild antimineralocorticoid properties. (Medicines.org.uk)

Antimicrobial Spectrum

None.

This medicine has no antibacterial, antiviral, or antifungal activity and does not protect against HIV or other sexually transmitted infections.

Pharmacokinetics

Drospirenone is rapidly and almost completely absorbed after oral administration, reaching peak serum concentrations in approximately 1–2 hours. Oral bioavailability is approximately 76–85%, and food does not materially affect bioavailability. (Medicines.org.uk)

Drospirenone is highly albumin bound and undergoes extensive metabolism.

Ethinylestradiol is rapidly absorbed but undergoes significant first-pass metabolism before systemic circulation.

Contraindications

Combined hormonal contraception is contraindicated in patients with current or previous venous thromboembolism, arterial thromboembolism, major thrombophilia, migraine with aura, severe hypertension, severe vascular diabetes, severe hepatic disease, hepatic tumours, hormone-dependent malignancy, or unexplained vaginal bleeding.

It is contraindicated during pregnancy.

Renal, hepatic, and adrenal disorders require additional consideration because Drospirenone has potassium-sparing activity.

Precautions

The principal serious safety concern is venous and arterial thromboembolism.

Risk assessment should include age, smoking, obesity, family history of thrombosis, major surgery, prolonged immobilisation, hypertension, and migraine.

Drospirenone may increase serum potassium in susceptible patients, particularly those with renal impairment or receiving potassium-increasing medicines.

Use During Pregnancy and Breastfeeding

Pregnancy: Contraindicated. Treatment should be discontinued if pregnancy is confirmed.

Breastfeeding: Combined oral contraceptives may reduce milk production and alter milk composition, particularly early postpartum.

Adverse Effects

Adverse effects may include headache, nausea, abdominal discomfort, breast tenderness, mood changes, breakthrough bleeding, and menstrual changes.

Rare but clinically important adverse reactions include DVT, pulmonary embolism, myocardial infarction, stroke, and hepatic disorders.

Hyperkalaemia may occur in susceptible patients.

Overdose

Overdose may cause nausea, vomiting, and withdrawal bleeding.

There is no specific antidote. Treatment is supportive and symptomatic.

Drug Interactions

Enzyme-inducing medicines may reduce contraceptive efficacy, including rifampicin, rifabutin, carbamazepine, phenytoin, phenobarbital, primidone, oxcarbazepine, and St. John’s Wort.

Combined hormonal contraceptives may reduce lamotrigine concentrations.

Potassium-increasing medicines such as spironolactone, eplerenone, potassium-sparing diuretics, ACE inhibitors, ARBs, and potassium supplements require additional caution.

Drug Name and Strength

Drospirenone 3 mg + Ethinylestradiol 30 micrograms per film-coated tablet

The specified BIOPHARM LIPZ 21 Tablets corresponds to the Thai-registered product LIPZ, containing Drospirenone 3 mg + Ethinylestradiol 30 mcg per active tablet, manufactured by Biolab Co., Ltd. and distributed by Biopharm.

Each pack contains 21 active light-yellow film-coated tablets and no placebo tablets. (MIMS)

Indications

Indicated for oral contraception to prevent pregnancy.

Thai LIPZ/LIPZ ED product information also notes potential additional benefits related to fluid-retention symptoms and acne, but oral contraception remains the principal indication.

Recommended Dosage

Take 1 tablet once daily at approximately the same time for 21 consecutive days.

This is followed by a 7-day tablet-free interval, during which withdrawal bleeding usually occurs. Start the next pack on Day 8, even if bleeding has not completely stopped. (Medicines.org.uk)

If no hormonal contraception was used during the preceding month, treatment may be started on Day 1 of menstrual bleeding.

If an active tablet is taken less than 12 hours late, contraceptive protection is generally maintained.

If more than 12 hours late, management depends on the number of missed tablets and the week of the cycle. The hormone-free interval should not exceed 7 days.

Warnings in Accordance with Public Safety Announcements

Combined hormonal contraceptives increase the risk of venous and arterial thromboembolism.

Smoking substantially increases cardiovascular risk, particularly in women over 35 years of age.

Patients with current or previous DVT/PE, major thrombophilia, migraine with aura, stroke, myocardial infarction, or multiple major cardiovascular risk factors require alternative contraception or specialist assessment.

Drospirenone has a potassium-sparing effect, so hyperkalaemia risk should be considered in susceptible patients.

This medicine does not protect against HIV or other sexually transmitted infections.

Storage Conditions

Store at not more than 30°C, in the original packaging, protected from excessive heat, moisture, and direct light.
